The Pathophysiological Basis of Differential Diagnosis and Typing of Hypertension Based on TCM
1.A total of 40 patients with essentialhypertension was divided into two clinicaltypes(each was subdivided into two groups)according to the principle of differen-tiating symptoms and signs based on tradi-tional Chinese medicine. In all the patients
24 hours urinary V.M.A.and 17 OHCS andblood lipid were measured. In 36 out of 40patients the microcirculation in nail-foldcapillaries was observed.2.Although mean V.M.A.and 17OHCSdid not appear to be abnormal when thepopulation of essential hypertension was con-sidered as a whole
several noteworthy fin-dings were apparent when comparisons weremade among the above mentioned clinicaltypes.Mean V.M.A and mean 17 OHCS inthe group of Yinxu-Yangkang(insufficiencyof vital essence and exuberance of vital func-tion)were significantly higher than that ofthe Yinxu-Yangxu(insufficient both in vitalenergy and vital function)
and it is interes-ring to see that in the two subgroups
theone with predominant exuberance of Yang
the other with predominant insufficiencyof Yang
the mean V.M.A.and mean 17 OHCSin the former were obviously higher thanthat of the latter. This results suggest that theclassification according to the principle oftraditional Chinese medicine coincide with themordern pathophysiological findings and thatthe sympatho-adrenergic system activity andhypothalamic-pituitary-adrenal axis activityare greater in Yinxu-Yangkang than Yinxu-Yangxu.These findings support the view thathyperactivity of the sympathetic nervous sys-tem plays an important role in the develop-ment of essential hypertension in some pati-ents and that its contribution may differ atdifferent stages of the disease.3.The incidence of the twisting of thecapillary loops in 11 patients with symptomsof“Blood Stasis”was higher than that of thecontrolled group
while the blood velocity ofthe former was slow than that of the latter.
