摘要

目的探讨宫清颗粒缩短早孕妇女药物流产后出血时间的可能机制。方法将非意愿妊娠49天内要求药物流产的宫内早孕妇女120例随机分成治疗组（米非司酮配伍米索前列醇药物流产同时给予宫清颗粒口服）、对照组（药物流产同时给予茜芷胶囊口服）、空白组（单纯药物流产）

每组40例;另选40例负压吸宫流产做对照（人流组）。检测自然排出绒毛及蜕膜组织中Bax、Bcl-2以及BaxmRNA、Bcl-2mRNA的表达。结果Bax、Bcl-2及BaxmRNA、Bcl-2mRNA在各组绒毛及蜕膜中均有表达。Bax、BaxmRNA在治疗组、对照组、空白组、人流组绒毛及蜕膜中的表达依次降低

Bcl-2、Bcl-2mRNA表达依次升高（P<0.05或P<0.01）;并且治疗组对蜕膜凋亡基因的影响尤为明显。结论宫清颗粒可以增强药物流产后绒毛及蜕膜中Bax的表达

抑制Bcl-2的表达

从而促进绒毛和蜕膜的细胞凋亡

减少蜕膜残留

这可能是其缩短药物流产后出血时间的机制之一。

Abstract

Objective To investigate the possible mechanism of Gongqing Granule（Granule for Clearing the Uterus） on shortening the duration of vaginal bleeding after medical abortion.Methods The 120 unwanted pregnancy women within 49d of intrauterine gestation were randomly divided into three groups.Gongqing Granule combined with mifepristone and misoprostol was administered orally in Group A（treatment group）.Qianzhi Capsule combined with mifepristone and misoprostol was administered orally to Group B（control group）

and only mifepristone plus misoprostol was administered orally to Group C（blank group）.Other 40 women（Group D

induced abortion group）

were treated with vacuum aspiration for abortion.Chorionic villi and decidual tissue were collected and expression of Bax and Bcl-2 were detected by immunohistochemical method and Bax mRNA and Bcl-2 mRNA by situ hybridization.Results The expression of Bax

Bcl-2

Bax mRNA and Bcl-2 mRNA was found in all groups.The expression of Bax and Bax mRNA in the treatment group was higher than those in the control group

then the blank group

and then induced abortion group.While the expression of Bcl-2 and Bcl-2 mRNA of the treatment group was lower than those in the control group

then the blank group

and then induced abortion group（P<0.05 or P <0.01）.And the effect on decidual apoptosis gene was especially significant in the treatment group.Conclusion Gongqing Granule can enhance the expression of Bax and decrease the expression of Bcl-2 to promote the apoptosis and reduce the decidual residue.This might be one of the mechanisms of Gongqing Granule in shortening the uterine bleeding after medical abortion.
