摘要

目的观察不同中医证型缺血性脑卒中患者的危险因素、临床特点和预后。方法197例急性缺血性脑卒中中经络患者入选

判定中医证型

并调研其危险因素

入院时采用简易精神状态检查表（MMSE）、美国国立卫生研究院卒中量表（NIHSS）、Barthel指数（BI）、改良Rankin量表（MRS）测定疾病状态

观察2周和3个月的NIHSS、BI、MRS的变化情况。结果不同中医证型脑卒中患者各危险因素的伴发率、脑卒中亚型的分布、入院时MMSE、NIHSS、BI、MRS差异均无统计学意义（P>0.05）;入院2周期间NIHSS的减分值、入院后3个月期间BI的增分值和MRS≤3患者的增加例数比例有明显统计学差异（P<0.05）。实证为主的中医证型NIHSS、BI、MRS优于虚证为主的中医证型。结论不同中医证型缺血性脑卒中中经络患者的近期预后不同。

Abstract

Objective To observe the risk factors

clinical characteristics and prognosis of ischemic stroke patients with different patterns of TCM syndrome.Methods Totally 197 acute ischemic stroke inpatients with meridian-collateral being attacked were enrolled in this study.Their patterns of TCM syndrome were diagnosed

and Mini-Mental State Examination（MMSE）

National Institute of Health Stroke Scale（NIHSS）

Barthel index（BI） and Modified Rankin Scale（MRS） on admission were used to evaluat the condition of the patients

then the changes in NIHSS

BI and MRS at 2-week and 3-month after admission were followed up as well.Results The prevalence of vascular risk factors

stroke subtypes

and MMSE

NIHSS

MRS at admission were not different significantly among stroke patients with different patterns of TCM syndrome（P>0.05）.However

the decreasing changes of NIHSS during the first 2 weeks

the increasing changes of BI and MRS（Rakin≤3） during the 3 months obviously differ among stroke patients with different patterns of TCM syndrome（P<0.05）.The effect of excess patterns of TCM syndrome was better than that of deficiency patterns of TCM syndrome.Conclusion Short-term prognosis of stroke was different in patients with different patterns of TCM syndrome.
