摘要

目的观察加减七厘散预防髋、膝关节置换术后下肢深静脉血栓（DVT）的临床疗效。方法将40例髋、膝关节置换术后患者随机分为治疗组和对照组各20例。治疗组手术前1天至术后6天口服加减七厘散

每次3g

每天2次;对照组于同时间皮下注射低分子肝素钠

每天0.4ml

两组均治疗7天。于术前1天和术后第2、4、6天检测两组患者血液流变学指标、血小板计数;术后3天观察切口引流总量;同时记录治疗期间不良反应

并观察DVT的发生情况。结果两组患者术后2天、4天、6天与本组术前1天比较血液流变学指标、血小板计数比较差异均有统计学意义（P<0.05）;治疗组术后2天、4天、6天与对照组同时间比较血液流变学指标、血小板计数比较差异无统计学意义（P>0.05）。治疗组术后3天切口引流总量明显低于对照组（P<0.05）。对照组发生不良反应共7例明显多于治疗组的1例（P<0.05）。两组DVT的发生率均为5%。结论加减七厘散可有效预防髋、膝关节置换术后下肢DVT的发生

在术后切口引流总量、不良反应率发生方面均低于低分子肝素钠。

Abstract

Objective To observe the therapeutic effect of Modified Qili Powder（Modified Seven Pinches Powder）in preventing postoperative deep vein thrombosis（DVT）after hip or knee replacement.Methods Forty patients after hip or knee replacement were randomized into the treatment group and control group

with 20in each.The treatment group was given 3g Modified Qili Powder twice daily from 1day before operation to 6days after operation.The control group was given subcutaneous injection of low molecular weight heparin 0.4ml daily.Both groups were treated for 7days.The blood rheology index and platelet count were detected 1day before operation and on the 2nd

4th and 6th day after operation.The wound drainage

adverse reactions and DVT were observed on the 3rd day after operation.Results Comparing with one day before operation

the blood rheology index and platelet count were significantly different on the 2nd

4th and 6th day after operation in both groups（P<0.05）.There was no significant difference between groups in the blood rheology index and platelet count on the2nd

4th and 6th day after operation（P>0.05）.Comparing with the control group

the total volume of wound drainage in 3days after operation was significantly lower in the treatment group（P<0.05）.There were 7cases of adverse reactions in the control group and 1case in the treatment group

with a significant difference between groups（P<0.05）.There was no significant difference between groups in the incidence of DVT（P>0.05）.Conclusion Modified Qili Powder can effectively prevent DVT after hip or knee replacement with a better effect in reducing the wound drainage and adverse reaction as compared with low molecular weight heparin.
