Objective To observe the efficacy and safety of Dihuang Yizhi Formula（ 地黄益智方） combined with Donepezil for Alzheimer’s disease（ AD） patients with kidney-essence deficiency and phlegm-stasis obstructing network vessels pattern. Methods Totally 100 AD patients with kidney-essence deficiency and phlegm-stasis obstructing network vessels pattern were randomized into the treatment group and control group

with 50 in each. Donepezil was given to both groups 5 to 10 mg per time

once daily. Dihuang Yizhi Formula（ 地黄益智方） was added to the treatment group one package daily. The treatment lasted for 6 months. The efficacy was evaluated with Mini-Mental State Examination（ MMSE）

Montreal Cognitive Assessment（ MOCA）

Alzheimer’s Disease Assessment Scale-Cognitive Part（ ADAS-Cog）

Activities of Daily Living（ ADL） Scale and TCM Symptom Score Scale. Results The scores of MMSE and MOCA were significantly increased but the scores of ADAS-Cog

ADL Scale and TCM Symptom Score Scale were significantly decreased in both groups after treatment（ P < 0. 05 or P < 0. 01）

especially in the treatment group（ P < 0. 01）. Conclusion Dihuang Yizhi Formula（ 地黄益智方） combined with Donepezil can obviously increase the cognitive ability

relieve the clinical symptoms and improve the ADL of AD patients with kidney-essence deficiency and phlegm-stasis obstructing network vessels pattern

with a better efficacy as compared with single application of Donepezil.
