摘要

目的观察破格救心汤结合心肺复苏救治心脏骤停的临床疗效。方法将60例心脏骤停患者分为治疗组和对照组各30例。对照组给予心肺复苏术等基础西医治疗;治疗组在对照组基础治疗开始时即鼻饲给予破格救心汤

每次100ml

每日4次

24h即止

两组均治疗至患者出院或死亡为止。观察自主循环恢复（ROSC）时间、ROSC率、院内存活率、院内生存时间

出院时记录格拉斯哥昏迷评分（GCS）

ROSC后24 h检测血清神经元特异性烯醇化酶（NSE）。结果两组患者院内存活率均为26.7%。治疗组患者ROSC率为86.7%

对照组为53.3%

治疗组明显高于对照组（P=0.005）。治疗组患者ROSC时间明显短于对照组（P=0.040）、院内生存时间明显长于对照组（P=0.028）。16例存活患者ROSC时间与出院时GCS呈负相关（Spearman相关系数=-0.555

P=0.032）。ROSC后24 h的NSE值与出院时GCS呈负相关（相关系数=-0.617

P=0.019）。结论破格救心汤结合心肺复苏救治心脏骤停患者能提高其ROSC率、缩短ROSC时间及延长院内生存时间。

Abstract

Objective To observe the therapeutic effect of Poge Jiuxin Decoction（ an unconventional decoction to save the heart

破格救心汤） combined with cardiopulmonary resuscitation（ CPR） for cardiac arrest. Methods Sixty cases of cardiac arrest were randomized into the treatment group and control group

with 30 in each. The control group was given CPR. The treatment group was given nasogastric feeding of Poge Jiuxin Decoction 100 ml for four times within 24 hours on the basis of CPR. Both groups were treated till discharge or death. The time of restoration of spontaneous circulation（ ROSC）

hospital survival rate and hospital survival time were observed. The Glasgow Coma Score（ GCS） was recorded at discharge. The neuron-specific enolase（ NSE） was detected 24 h after ROSC. Results The hospital survival rate was 26. 7% in both groups. The ROSC rate was 86. 7% in the treatment group and was significantly higher than 53. 3% in the control group（ P = 0. 005）. The ROSC time in the treatment group was significantly shorter than that in the control group（ P = 0. 040）. P = 0. 019）. Conclusion Poge Jiuxin Decoction combined with CPR can increase the ROSC rate

shorten ROSC time and prolong the hospital survival time in patients with cardiac arrest.
