Objective To observe the therapeutic effect of Qianjiang Granules for hypertension with diastolic heart failure（ DHF）. Methods Totally 78 cases of hypertension with DHF were randomized into the treatment group and control group

with 39 in each. The control group was given routine chemical medicine. The treatment group was added with Qiangjiang Granules. The efficacy of TCM syndromes in both groups were observed after 8-week treatment. The levels of N-terminal pro-B-type natriuretic peptide（ NT-proBNP）

left atrial diameter（ LAD）

left ventricular end systolic diameter（ LVESD）

left ventricular end-diastolic diameter（ LVEDD）

interventricular septum diameter（ IVSD）

left ventricular posterior wall diameter（ LVPWD） and mitral ratio of peak early to late diastolic filling velocity（ E /A） were compared in both groups before and after treatment. Results The total effective rate of TCM syndromes in the treatment group was superior to that in the control group（ P < 0. 05）. The NT-proBNP level was significantly decreased in both groups after treatment（ P < 0. 01）

especially in the treatment group（ P <0. 05）. The levels of LVEDD

IVSD and LVPWD in both groups and LAD in the treatment group were significantly decreased after treatment（ P < 0. 01） but E /A was significantly increased（ P < 0. 01）. The LAD level in the treatment group was significantly lower than that in the control group after treatment（ P < 0. 05）. The levels of systolic blood pressure

diastolic blood pressure and pulse pressure were significantly decreased in both groups after treatment（ P <0.05 or P <0.01）

especially systolic blood pressure in the treatment group（ P <0. 05）. Conclusion The combo therapy of Qianjiang Granules and chemical medicine can improve symptoms and heart function of hypertension with DHF patients.
