On the basis of 21 items in Minnesota Living with Heart Failure Questionnaire（ MLHF）

the authors added 16 items of Chinese medical characteristic related to chronic cardiac failure that been selected by comprehensive literature and experts to form pre-survey scale with 37 items and chose 100 patients with chronic cardiac failure to pre-survey. On the basis of analyzing and summarizing results of pre-survey

the authors expanded the scope to consult revise and supplemented opinions from experts to form item pool of test questionnaire including 39 items of MLHF and characteristic items of Chinese medicine

then chose 130 patients with chronic cardiac failure to test investigation.Processed by tendency of dispersion method

correlation coefficient method and stepwise regression analysis method

deleted items of low relevance and initially formed Modified MLHF（ C-MLHF）including 28 items based on Chinese culture. The C-MLHF

accorded with the life habit of countrymen and developed based on Chinese culture and background of Chinese medicine theory and according to universal programmed way of international scale

has better adaptability to evaluate quality of life of patients with cardiac failure in China.
