摘要

目的观察补肾健脾、解毒利咽中药治疗IgA肾病的临床疗效。方法将60例IgA肾病患者随机分为治疗组和对照组各30例

治疗组给予补肾健脾、解毒利咽中药汤剂口服

每日1剂;对照组给予洛汀新10 mg

每日1次口服

两组疗程均为3个月。比较两组治疗前后中医证候积分、24 h尿蛋白定量、尿沉渣红细胞计数及不良反应。结果治疗组脱落1例

对照组脱落3例。治疗后治疗组中医证候中腰脊酸痛、咽喉肿痛、疲倦乏力、浮肿、纳少、口干积分较治疗前降低（P<0.05）;对照组治疗前后各项症状积分均无明显变化（P>0.05）。与治疗前比较

治疗后治疗组患者24 h尿蛋白定量及尿红细胞计数均明显减少（P<0.01）;对照组治疗后仅24 h尿蛋白定量明显降低（P<0.05）

尿红细胞计数差异无统计学意义（P>0.05）。两组治疗后比较

治疗组24 h尿蛋白定量及尿红细胞计数均明显低于对照组（P<0.05或P<0.01）。两组患者治疗前后血、便常规

肝、肾功能

心电图等未见异常改变。结论补肾健脾、解毒利咽中药能有效改善Ig A肾病患者的临床症状

降低24 h尿蛋白定量及尿沉渣红细胞计数

安全有效。

Abstract

Objective To observe the clinical effect of herbs of reinforcing kidney

strengthening spleen

detoxifying and relieving sore throat on IgA nephropathy. Methods Sixty patients with IgA nephropathy were randomized into a treatment group（ 30 cases） and a control group（ 30 cases）. The treatment group was given herbal decoction of reinforcing kidney

strengthening spleen

detoxifying and relieving sore throat

one dose every day. The control group was given 10 mg of lotensin orally

once every day. The treatment course was 3 months for the both groups. Scores of Chinese medicine syndrome

24 hours urinary protein excretion

urinary sediment red blood cell count and side effect were compared in both groups before and after treatment. Results One case dropped out from the treatment group

and 3 cases from the control group. Symptom scores of back pain

sore throat

hypodynamia

edema

poor appetite and dry mouth decreased in the treatment group after treatment（ P < 0. 05）. Symptom scores changed little in the control group after treatment（ P > 0. 05）. After treatment

24 hours urinary protein excretion and urinary sediment red blood cell count decreased obviously in the treatment group（ P < 0. 05）; but only 24 hours urinary protein excretion decreased obviously in the control group（ P < 0. 05）. There was no significant difference in urinary sediment red blood cell count before and after treatment in the control group（ P > 0. 05）. After treatment

24 hours urinary protein excretion and urinary sediment red blood cell count in the treatment group were much lower than those in the control group（ P < 0. 05 or P < 0. 01）. Blood routine

hepatic and renal function

as well as electrocardiogram of patients in both groups before and after treatment did not show abnormity. Conclusion Herbs of reinforcing kidney

strengthening spleen

detoxifying and relieving sore throat could improve the clinical symptoms of IgA nephropathy patients and decrease 24 hours urinary protein excretion and urinary sediment red blood cell count effectively. The treatment seems safe and effective.
