Relationship between Barrett Esophageal Lesions under Narrow Band Imaging Endoscope and Chinese Medicine Syndrome of Stagnation of Liver Qi
Objective To explore the distribution characteristics and the dependency with columnar epithelial metaplasia of syndrome of stagnation of liver Qi and subtypes of patients with Barrett esophageal lesions under narrow band imaging( NBI) endoscope. Methods Two hundred and thirty patients with Barrett esophageal lesions
diagnosed by endoscope and pathology
were collected. After Chinese medicine syndrome differentiation
105 cases were syndrome of stagnation of liver Qi
125 cases not. The syndrome of stagnation of liver Qi included 6 subtypes
such as syndrome of incoordination between liver and stomach
syndrome of heat stagnation in liver and stomach
syndrome of stagnation of liver Qi and coagulated phlegm
syndrome of stagnation of liver Qi and blood stasis
syndrome of stagnation of liver Qi and injury of Yin
as well as syndrome of stagnation of liver Qi and spleen deficiency. Pit pattern typing and columnar epithelial metaplasia length of patients of all syndromes was examined by NBI endoscope under normal mode and amplification mode respectively. The dependency between subtypes of syndrome of stagnation of liver Qi and columnar epithelial metaplasia length was analyzed by corresponding correlation analysis. Results Among105 patients with syndrome of stagnation of liver Qi
under NBI normal mode
island-type accounted for 84. 76%
tongue-type for 12. 38%
circumference-type for 2. 85%. Under amplification mode
pit-1 and pit-2 were the main pit pattern types. The difference of distribution of island-type
tongue-type and circumference-type between the syndrome of stagnation of liver Qi and non-syndrome of stagnation of liver Qi was significant( P < 0. 01). The difference of distribution of pit pattern types between the syndrome of stagnation of liver Qi and non-syndrome of stagnation of liver Qi was significant( P < 0. 05). The syndrome of incoordination between liver and stomach( 36 cases)
syndrome of heat stagnation in liver and stomach( 29 cases) and syndrome of stagnation of liver Qi and spleen deficiency( 27 cases) were the main subtypes of the syndrome of stagnation of liver Qi. The syndrome of incoordination between liver and stomach had the most columnar epithelial metaplasia of short segment( correlation coefficient 0. 1186); the syndrome of stagnation of liver Qi and coagulated phlegm had the most columnar epithelial metaplasia of long segment( correlation coefficient 0. 1427). Conclusion Under the NBI technology
island-type was the most common in patients with Barrett esophageal lesions of the syndrome of stagnation of liver Qi. Pit-1 was the most common pit pattern type in patients of the syndrome of incoordination between liver and stomach and the syndrome of heat stagnation in liver and stomach. Pit-2 was the most common pit pattern type in patients of the syndrome of stagnation of liver Qi and spleen deficiency and the syndrome of incoordination between liver and stomach. Columnar epithelial metaplasia of short segment and the syndrome of incoordination between liver and stomach had correlation. Columnar epithelial metaplasia of long segment and the syndrome of stagnation of liver Qi and coagulated phlegm had correlation.
