摘要

目的总结针刺治疗脑卒中后吞咽障碍的取穴规律。方法检索中国期刊全文数据库2012年1月至2016年12月针刺治疗脑卒中后吞咽障碍临床随机对照试验研究

提取处方穴位建立针刺治疗脑卒中后吞咽障碍文献数据库

并分别对纳入穴位进行使用频数、高频穴位聚类分析及穴位组合关联规则分析。结果共纳入处方105首

穴位83个。单穴位使用频数位于前3位的分别为廉泉（71）、风池（66）、金津（48）、玉液（48）。对出现频数≥5的穴位进行样本聚类

结果显示针刺治疗脑卒中后吞咽障碍处方穴位聚为四类较好。两穴位关联规则支持度位于前3位的分别为金津玉液（45.71%）、玉液金津（45.71%）

风池翳风（32.38%）

风池完骨（19.05%）、翳风完骨（19.05%）;三穴位关联规则支持度位于前3位的分别为金津玉液廉泉（35.24%）、玉液金津廉泉（35.24%）

金津玉液风池（32.38%）、玉液金津风池（32.38%）、廉泉玉液风池（32.38%）、廉泉金津风池（32.38%）

风池翳风廉泉（22.86%）;四穴位关联规则支持度位于前3位的分别为廉泉玉液金津风池（32.38%）

金津玉液廉泉风池（27.62%）、玉液金津廉泉风池（27.62%）

廉泉翳风玉液金津（16.19%）、风池翳风玉液金津（16.19%）。结论针刺治疗脑卒中后吞咽障碍选穴主要体现穴位近治作用

廉泉穴为核心穴位

金津、玉液

金津、玉液、廉泉

金津、玉液、廉泉、风池等为常用穴位组合。

Abstract

Objective To summarize the point selection rules of acupuncture treatment of dysphagia after stroke.Methods Clinical randomized controlled trials of acupuncture treatment of dysphagia after stroke published from January 2012 to December 2016 were searched in Journal Full-Text Database of China National Knowledge Infrastructure. A literature database of acupuncture treatment of dysphagia after stroke was established based on the information of acupuncture prescriptions extracted from the search results. Frequency analysis of acupoint application

high frequency-acupoints cluster analysis and association rules analysis of point combinations were conducted. Results Totally

105 acupuncture prescriptions were included

with 83 acupoints. Top 3 single points with high use frequency were Lianquan（ RN 23）（ 71）

Fengchi（ GB 20）（ 66）

and Jinjin（ EX-HN 12）（ 48） and Yuye（ EX-HN 13）（ 48）. Sample cluster analysis was made on points with the frequency more than 5. The result showed that the points in the acupuncture prescriptions for dysphagia after stroke could be clustered into 4 categories. Top 3 point-pairs with high support degree of association rule were Jinjin（ EX-HN 12）-Yuye（ EX-HN 13）（ 45. 71%）

Fengchi（ GB 20）-Wangu（ GB 12）（ 19. 05%） and Yifeng（ TE 17）-Wangu（ GB 12）（ 19. 05%）. The top 3 three-point groups with high support degree of association rule were Jinjin（ EX-HN 12）-Yuye（ EX-HN 13）-Lianquan（ RN 23）（ 35. 24%）

Jinjin（ EX-HN 12）-Yuye（ EX-HN13）-Fengchi（ GB 20）（ 32. 38%）

Yuye（ EX-HN 13）-Jinjin（ EX-HN 12）-Fengchi（ GB 20）（ 32. 38%）

Lianquan（ RN 23）-Yuye（ EX-HN 13）-Fengchi（ GB 20）（ 32. 38%）

Lianquan（ RN 23）-Jinjin（ EX-HN12）-Fengchi（ GB 20）（ 32. 38%） and Fengchi（ GB 20）-Yifeng（ TE 17）-Lianquan（ RN 23）（ 22. 86%）. The top 3 four-point groups with high support degree of association rule were Lianquan（ RN 23）-Yuye（ EX-HN 13）-Jinjin（ EX-HN 12）-Fengchi（ GB 20）（ 32. 38%）

Jinjin（ EX-HN 12）-Yuye（ EX-HN 13）-Lianquan（ RN23）-Fengchi（ GB 20）（ 27. 62%）

Yuye（ EX-HN 13）-Jinjin（ EX-HN 12）-Lianquan（ RN 23）-Fengchi（ GB20）（ 27. 62%）

Lianquan（ RN 23）-Yifeng（ TE 17）-Yuye（ EX-HN 13）-Jinjin（ EX-HN 12）（ 16. 19%） and Fengchi（ GB 20）-Yifeng（ TE 17）-Yuye（ EX-HN 13）-Jinjin（ EX-HN 12）（ 16. 19%）. Conclusion Point selection in acupuncture treatment of dysphagia after stroke indicates that the local points play a major role. Lianquan（ RN 23） is the core point. Jinjin（ EX-HN 12）-Yuye（ EX-HN 13）

Jinjin（ EX-HN 12）-Yuye（ EX-HN 13）-Lianquan（ RN 23） and Jinjin（ EX-HN 12）-Yuye（ EX-HN 13）-Lianquan（ RN 23）-Fengchi（ GB 20） are frequently-used point combinations.
