Clinical Observation on 30 Cases of Hepatitis B at Early Stage of Acute-on-chronic Liver Failure Treated by Modified Jiedu Liangxue Lishi Fang (解毒凉血利湿方) Combined with Western Medicine
Objective To observe the efficacy of integrative Chinese and western medicine treatment program based on the method of resolving toxin
cooling the blood and draining dampness in the treatment of patients with hepatitis B and acute-on-chronic liver failure at early stage. Methods Sixty-three patients with hepatitis B and acuteon-chronic liver failure at early stage were randomized into integrative medicine group( 30 cases) and western medicine control group( 33 cases). The western medicine control group was given comprehensive treatment of western medicine
in addition
the integrative medicine group was given Jiedu Liangxue Lishi Fang( toxin resolving
blood cooling and dampness draining decoction
解毒凉血利湿方) for 4 weeks and the patients were followed up for 8 weeks. The changes of hepatic function and coagulation function indexes in both groups before and after treatment were detected. The number of cases progressing to acute-on-chronic liver failure and the cumulative effective cases at different time after treatment were counted
based on which the disease progression rate and the cumulative effective rate were calculated. Results The progression rates of the integrative medicine group were all 6. 67% in the 2 nd
3 rd
4 th week and the 2 nd and 4 th follow-up week
respectively
while that of the western medicine treatment group was24. 24%. The cumulative effective rate of the integrative medicine group was 70. 00% in the 2 nd week while that of the western medicine control group was 45. 45%
with significant difference between groups( P < 0. 05). Compared with those before treatment
the levels of alanine aminotransferase and aspartate aminotransferase decreased in both groups in the 2 nd week
while in the integrative medicine group
total bilirubin( Tbi L) and total bile acid( TBA) in the 2 nd week
alkaline phosphatase in the 8 th follow-up week and glutamyl-transpeptidase in the 2 nd
4 th and 8 th follow-up weeks decreased; compared with the western medicine control group
the TBi L and TBA in the integrative medicine group decreased in the 2 nd and 3 rd weeks( P < 0. 05 or P < 0. 05). Compared with those before treatment
the white cell count and neutrophil count( NC) decreased in the integrative medicine group in the 4 th week and the4 th and 8 th follow-up weeks after treatment
and the neutrophil lymphocyte ratio( NLR) decreased in the 2 nd week;compared with those in the western medicine control group
the NC and NLR in the integrative medicine group decreased in the 1 st and 2 nd weeks after treatment
and the lymphocyte count increased in the 4 th and 8 th follow-up weeks( P < 0. 05 or P < 0. 01). Conclusion The integrative medicine treatment program combined Chinese and western medicine based on the method of resolving toxin
cooling the blood and draining dampness can reduce the progression rate of hepatitis B patients with acute-on-chronic liver failure in early stage
improve the treatment efficiency and improve the prognosis of patients.
