Clinical Observation on 310 Cases of Chronic Heart Failure with Qi Deficiency and Blood Stasis Syndrome Treated by Shenkui Tongmai Granules ( 参葵通脉颗粒)
Objective To evaluate the clinical effect and the possible mechanism of Shenkui Tongmai Granules( 参葵通脉颗粒) in treating chronic heart failure with Qi deficiency and blood stasis syndrome. Methods Six hundred and twenty chronic heart failure patients with Qi deficiency and blood stasis syndrome were randomly divided into a treatment group and a control group
with 310 cases in each group. The two groups were both treated with basic treatment. The treatment group was additionally treated with Shenkui Tongmai Granules
while the control group was additionally given the placebo of Shenkui Tongmai Granules
both one bag each time
twice a day for 8 weeks. The cardiac function( including left ventricular ejection fraction( LVEF)
left ventricular end systolic diameter( LVDs)
and left ventricular end diastolic diameter( LVDd))
quality of life score
cardiac function classification
and 6-minute walking distance were compared between the two groups before and after treatment. Plasma levels of N-terminal pro brain natriuretic peptide( NT-pro BNP)
nitric oxide( NO)
angiotensin Ⅱ( Ang Ⅱ)
tumor necrosis factor-α( TNF-α) and malondialdehyde( MDA) were measured. The effects on cardiac function classification and Chinese medicine syndrome were determined. Results The total effective rate of cardiac function classification was 90. 32%in the treatment group and 80. 32% in the control group. The total effective rate of Chinese medicine syndrome was 92. 90% in the treatment group and 61. 94% in the control group. The curative effects of heart function classification and Chinese medicine syndrome in the treatment group were both better than those in the control group( P < 0. 05).The 6-minute walking distance
LVEF and the level of NO increased significantly
while the levels of NT-pro BNP
Ang Ⅱ
TNF-α and MDA
LVDs
LVDd
quality of life score decreased significantly in the treatment group after treatment( P < 0. 05)
and the improvement in the treatment group was better than that in the control group( P <0. 05). Conclusion Shenkui Tongmai Granules seem to have good clinical effect in the treatment of chronic heart failure with Qi deficiency and blood stasis syndrome. Shenkui Tongmai Granules could improve cardiac function and quality of life. The mechanism may be related to the regulation of neuroendocrine disorders and myocardial remodeling in patients.
