摘要

目的观察散瘀消银汤综合疗法治疗重度斑块状银屑病的临床疗效。方法将90例重度斑块状银屑病患者随机分为治疗组和对照组各45例

对照组给予中药药浴配合NB-UVB照射、卡泊三醇软膏外涂

治疗组在对照组基础上配合散瘀消银汤口服

每日1剂

4周为1个疗程

共治疗3个疗程。治疗前及治疗2、3个疗程后观察银屑病皮损面积、评定严重程度指数（PASI）评分

并判定临床疗效

6个月后随访复发情况。结果治疗组临床疗效总有效率为93.3%（42例/45例）

对照组总有效率为75.6%（34例/45例）

治疗组优于对照组（P<0.05）。治疗2个疗程后及3个疗程后

治疗组PASI评分明显低于对照组（P<0.05）。与本组治疗前比较

治疗2个疗程后两组PASI评分均低于治疗前（P<0.05）。治疗3个疗程后

两组PASI评分均低于治疗2个疗程后（P<0.05）。治疗组未见明显不良反应

对照组个别出现红斑、瘙痒症状

减少NB-UVB照射剂量后逐渐缓解。结论散瘀消银汤综合疗法治疗重度斑块状银屑病的临床疗效确切

复发率低

未见明显不良反应。

Abstract

Objective To observe the clinical efficacy of combined therapy of Sanyu Xiaoyin Decoction（散瘀消银汤） in the treatment of severe plaque psoriasis. Methods Totally 90 cases of severe plaque psoriasis treated in our department from April 2013 to December 2016 were randomly divided into 45 cases respectively in the treatment group and the control group. The control group was treated with traditional Chinese medicine bath combined with NBUVB irradiation

coated with Calcipotriol Ointment; Treatment group was added Sanyu Xiaoyin Decoction. The changes of PASI during the treatment were observed and 4 weeks were 1 courses. A total of 3 courses were observed

and the recurrence was followed up after half a year. Results The total effective rate of the treatment group was93. 3%

and the total effective rate of the control group was 75. 6%. The treatment group was better than the control group（ P < 0. 05）. After 2 courses and 3 courses of treatment

PASI score of the treatment group was significantly lower than the control group（ P < 0. 05）. Compared with before treatment

after 2 courses of treatment

the score of PASI in both the treatment group and the control group was lower than that before the treatment（ P < 0. 05）. After 3 courses of treatment

the scores of PASI in both the treatment group and the control group were all lower than that of the 2 courses（ P < 0. 05）. There was no obvious adverse reaction in the treatment group

and the control group showed the symptoms of erythema and pruritus

and decreased gradually after reducing the dose of NB-UVB irradiation. Conclusion The combined therapy of Sanyu Xiaoyin Decoction is effective and safe in the treatment of severe plaque psoriasis

with low relapse rate. It is worthy of popularization and application.
