摘要

目的探讨保肾片对冠状动脉造影围手术期患者术后炎症反应及急性肾损伤的预防作用及可能机制。方法选取冠状动脉造影围手术期患者80例

随机分为治疗组40例（冠状动脉介入治疗术+标准基础治疗+保肾片）和对照组40例（冠状动脉介入治疗术+标准基础治疗）。分别于术前、术后24 h、术后72 h检测两组患者血清肌酐（SCr）及炎症因子超敏C反应蛋白（hs-CRP）、肿瘤坏死因子α（TNF-α）、白细胞介素6（IL-6）、白细胞介素8（IL-8）水平。结果术后24 h治疗组TNF-α、IL-6、IL-8明显低于对照组（P<0.05）;术后72 h治疗组SCr、hs-CRP、IL-8明显低于对照组（P<0.05）。治疗组术后24 h hs-CRP水平明显高于术前

TNF-α水平明显低于术前（P<0.05）;术后72 h SCr、hs-CRP、IL-6、IL-8水平均明显低于术前（P<0.05）。对照组术后24 h hs-CRP、TNF-α、IL-6、IL-8水平明显高于术前（P<0.05）;术后72 h SCr、hs-CRP水平明显高于术前（P<0.05或）。结论保肾片在冠状动脉造影围手术期的应用能够降低术后Scr水平及机体炎症反应的发生

进而保护和修复造影剂诱导的急性肾脏损伤。

Abstract

Objective To evaluate the efficacy and possible mechanisms of preventing the inflammatory reaction and the early acute renal injury by using Baoshen Tablets（保肾片） in patients underwent coronary angiography.Methods A total of 80 patients with coronary artery disease who underwent percutaneous coronary interventions（ PCI） were selected in this study. All subjects were randomly divided into treatment group（ 40 cases

coronary artery operation + routine treatment + Baoshen Tablets） and control group（ 40 cases

coronary artery operation + routine treatment）. The serum creatine（ Scr） and inflammatory factors including hypersensitive C-reactive protein（ hsCRP）

tumor necrosis factor-α（ TNF-α）

interleukin-6（ IL-6）、interleukin-8（ IL-8） were recorded and compared at pre-operation

24 h and 72 h after PCI operation in both groups. Result Based on the contrasting analysis among two different groups

at 24 hours after PCI treatment

the levels of TNF-α、IL-6、IL-8 in the treatment group were lower than those in the contrast group（ P < 0. 01 or P < 0. 05）. At 72 hours after PCI treatment

the levels of Scr

hs-CRP、IL-8 in the treatment group were lower than those in the contrast group（ P < 0. 01 or P < 0. 05）. After comparing in the treatment group

at 24 hours after PCI treatment

the level of hs-CRP was higher than that at pre-operation（ P < 0. 01）

and the level of TNF-α was lower than that at pre-operation（ P < 0. 05）. At 72 hours after PCI treatment

the levels of Scr、hs-CRP、IL-6、IL-8 t were all lower than those at pre-operation（ P < 0. 01 or P <0. 05）. After comparing in the contrast group

at 24 hours after PCI treatment

the levels of hs-CRP、TNF-α、IL-6、IL-8 were higher than those at pre-operation（ P < 0. 01 or P < 0. 05）. At 72 hours after PCI treatmen

the levels of SCr and hs-CRP were higher than those at pre-operation（ P < 0. 01 or P < 0. 05）. Conclusion The Baoshen Tablets can decrease the secretion of various proinflammatory cytokines

and reduce elevation of serum concentration. This specially formulated product has shown the protection and prothetic capability in for contrast-induced acute kidney injury.
