摘要

目的观察黄芪泽苓颗粒口服联合逐水膏脐贴辅助治疗中重度乙型肝炎肝硬化腹水的临床疗效。方法 96例中重度乙型肝炎肝硬化腹水患者随机分为治疗组与对照组各48例。对照组患者采用西医常规疗法治疗

治疗组患者在此基础上加用黄芪泽苓颗粒口服及逐水膏穴位贴敷神阙穴治疗

两组均治疗8周。观察两组患者治疗前后临床症状

检测肝功能、血氨及血清内毒素（LPS）、内皮素1（ET-1）、一氧化氮（NO）水平

彩超检查腹水深度、门静脉内径、脾脏厚度变化

并记录患者并发症及不良反应发生情况

分析平均腹水超滤治疗次数和平均住院治疗时间

治疗后判断临床综合疗效。结果治疗组临床综合疗效总有效率为93.75%（45/48）

对照组总有效率为72.92%（35/48）

治疗组高于对照组（P<0.05）;两组患者治疗后肝功能指标均明显改善

血氨和血清LPS、ET-1、NO水平降低

腹水深度和脾脏厚度亦降低

且治疗组改善优于对照组（P<0.05）。治疗组并发感染、消化道出血、肝性脑病及肝肾综合征发生率分别为12.5%、2.08%、4.17%、0

对照组分别为27.08%、12.5%、22.92%、4.17%

治疗组均低于对照组（P<0.05）。治疗组平均腹水超滤治疗次数和平均住院治疗时间为均低于对照组（P<0.05）。结论黄芪泽苓颗粒口服联合逐水膏脐贴辅助治疗中重度乙型肝炎肝硬化腹水能够加速患者腹水消退

促进肝功能恢复

减少并发症发生。

Abstract

Objective To observe the clinical effects of oral administration of Huangqi Zeling Granule（黄芪泽苓颗粒） combined with navel paste of Zhushui Cream（逐水膏） in treating moderate and severe hepatitis B cirrhosis ascites. Methods A total of 96 cirrhotic ascites patients with moderate and severe hepatitis B were randomly divided into treatment group and control group

with 48 cases in each group. The patients in the control group were treated with western medicine routine treatment. The patients in the treatment group were added Huangqi Zeling Granules orally and Zhushui Cream on Shenque（CV8）. Both groups were treated for 8 weeks. The changes of clinical symptoms of two groups before and after treatment were observed. The liver function

and the levels of blood ammonia（AMON）

lipopolysaccharide（LPS）

endothelin-1（ET-1）

nitric oxide（NO） were detected. The color Doppler examination was used to detect ascites depth

portal diameter and spleen thickness. The incidence of complications and adverse reactions was recorded. The average number of ascites ultrafiltration treatment and the average hospitalization time were analyzed

and the clinical comprehensive effect was judged after treatment. Results The total effective rate was 93. 75%（45/48） in the treatment group and 72. 92%（35/48） in the control group

and the treatment group was obviously superior to the control group（P < 0. 05）; The liver function index of 2 groups were significantly improved. The AMON

LPS

ET-1 and NO levels were decreased. The depth of ascites and thickness of the spleen were also decreased

and the treatment group was superior to the control group（P < 0. 05）. The incidence of complications

Conclusion Oral administration of Huangqi Zeling Granule and navel paste of Zhushui Cream in the treatment of moderate and sever hepatitis B cirrhosis ascites can accelerate the withdrawal of ascites

promote the recovery of liver function

and reduce the complications.
