摘要

目的观察补肺活血胶囊联合化疗治疗初治结核性胸膜炎阴虚内热证的临床疗效和安全性。方法128例初治结核性胸膜炎阴虚内热证患者随机分为治疗组62例和对照组66例

治疗组采用西医标准化疗方案（2HREZ/10HRE）联合补肺活血胶囊

对照组单用西医标准化疗方案

两组疗程均为2个月。观察两组胸腔积液排出总量及胸腔积液吸收时间;治疗前及治疗1周检测胸腔积液中肿瘤坏死因子α（TNF-α）水平;治疗前及治疗1、2个月观察胸膜厚度和肺功能情况

治疗后判定临床疗效

并记录各组不良反应。结果治疗组胸腔积液的总排出量以及胸腔积液吸收时间均小于对照组（P <0. 05）;治疗1周后两组TNF-α水平均较本组治疗前下降

且治疗组显著低于对照组（P <0. 05）。两组治疗1、2个月胸膜厚度与治疗前比较均明显减小（P <0. 05）;且治疗2个月后

治疗组胸膜厚度小于对照组（P <0. 05）。两组治疗1、2个月第1秒用力呼气容积/用力肺活量（FEV1/FVC）较治疗前明显降低

而肺活量（VC）较治疗前明显升高（P <0. 05）

治疗2个月后治疗组FVC较治疗前升高（P <0. 05）。治疗1个月治疗组VC高于同期对照组（P <0. 05）;治疗2个月治疗组FEV1/FVC低于同期对照组、VC高于同期对照组（P <0. 05）。治疗组治愈率为45. 2%

明显高于对照组的27. 3%（P <0. 05）;两组临床疗效总有效率差异无统计学意义（P> 0. 05）。治疗组不良反应发生率低于对照组（P <0. 05）。结论补肺活血胶囊辅助西医化疗治疗初治结核性胸膜炎阴虚内热证

可促进胸腔积液的吸收

降低胸腔积液中的TNF-α浓度

改善胸膜肥厚与肺功能

且能降低不良反应发生率。

Abstract

Objective To evaluate the efficacy and safety of Bufei Huoxue Capsule（补肺活血胶囊） combined with chemotherapy for the primary treatment of tuberculous pleuritis patients with yin deficiency and internal heat syndrome. Methods A total of 128 primary treatment of tuberculous pleuritis with yin deficiency and internal heat syndrome were randomly divided into a treatment group（ 62 cases） and a control group（ 66 cases）. Western medicine standard chemotherapy（ 2 HREZ/10 HRE） combined with Bufei Huoxue Capsule was used in the treatment group

and western medicine standard chemotherapy only was used in the control group

both for continuously 2 months. The volume and absorption time of pleural effusion

TNF-α levels in pleural effusion before treatment and after 1 week treatment

pleural thickness and lung function before treatment and after 1

2 months treatment

and the adverse reaction during the treatment were all recorded. Clinical efficacy of both groups was calculated after treatment. Results The sputum volume and absorption time of pleural effusion in the treatment group were significantly smaller than those in the control group（ P < 0. 05）. The TNF-α level in the treatment group after 1 week treatment was significantly lower than that in the control group（ P < 0. 05）

and the level of the treatment group was more lower than that of the control group. The pleural thickness

lung function of the two groups were significantly improved after treatment respectively（ P < 0. 05）; and the treatment group was superior to the control group after 2 months treatment（ P < 0. 05）.The cure rate of the treatment group was 45. 2%

which was higher than that of the control group（ 27. 3%）（ P <0. 05）. There was no significant difference in total effective rate between groups. The incidence of adverse reactions in the treatment group was lower than that in the control group（ P < 0. 05）. Conclusion The anti-TB program combined with Bufei Huoxue Capsule is effective and safe for primary treatment of tuberculous pleuritis with yin deficiency and internal heat syndrome. It can shorten the course of disease

promote the absorption of pleural effusion

reduce the pleural thickness and improve the lung function.
