摘要

目的观察冠心丹参滴丸治疗老年冠心病不稳定型心绞痛气虚血瘀证的临床疗效

并探讨可能的作用机制。方法 240例老年冠心病不稳定型心绞痛气虚血瘀证患者随机分治疗组和对照组各120例。在西医综合及对症治疗基础上

治疗组口服冠心丹参滴丸

每次0.4g

每日3次

对照组给予同等剂量的安慰剂口服

共治疗4周。治疗后观察两组心绞痛症状疗效、心电图疗效

治疗前后进行西雅图心绞痛量表（SAQ）评分和全球急性冠脉事件注册危险（GRACE）评分

检测外周血血小板微粒（PMPs）及其表面膜蛋白（CD40L、CD62p、CD63、PAC-1）水平

血清金属基质蛋白酶9（MMP-9）、可溶性细胞间黏附分子1（sICAM-1）、1型组织纤溶酶原激活物抑制剂（tPAI-1）水平

并观察不良反应。结果治疗组脱落2例

对照组脱落4例

治疗组与对照组分别有118例、116例完成研究。治疗组心绞痛症状疗效、心电图疗效总有效率分别为94.07%、87.29%

明显高于对照组的82.76%、76.72%（P<0.05）。治疗后两组SAQ评分均升高

GRACE评分均降低

外周血PMPs、CD40L、CD62p、CD63、PAC-1及血清MMP-9、sICAM-1、tPAI-1水平亦降低（P<0.05）

且治疗组各指标改善优于对照组（P<0.05）。结论冠心丹参滴丸治疗老年冠心病不稳定型心绞痛气虚血瘀证可提高临床疗效

其机制可能与抑制血小板活化及降低血清MMP-9、sICAM-1、tPAI-1水平有关。

Abstract

Objective To observe the clinical effects of Guanxin Danshen Dripping Pills（冠心丹参滴丸） in the treatment of elderly patients with unstable angina pectoris of qi deficiency and blood stasis

and to explore the possible mechanism. Methods Totally 240 elderly patients with unstable angina pectoris of qi deficiency and blood stasis were randomly divided into a treatment group and a control group

with 120 cases in each group. On the basis of comprehensive and symptomatic treatment of western medicine

the treatment group was given Guanxin Danshen Dripping Pills

0.4 g each time

3 times a day

and the control group was given an oral placebo at the same dose for 4 weeks. After treatment

the efficacy of angina pectoris symptoms and electrocardiogram were observed in both groups. Seattle angina questionnaire（SAQ） score and global registry of acute coronary event（GRACE） risk score were measured before and after treatment. Peripheral blood platelet particles（PMPs） and level of surface membrane proteins CD40 L

serum metalloproteinase 9（MMP-9）

soluble intercellular adhesion molecule 1（sICAM-1）

type 1 tissue plasminogen activator inhibitor（tPAI-1） level

and adverse reactions were observed. Results Two cases in the treatment group and 4 cases in the control group fell out

respectively. The effective rate of angina pectoris symptom and electrocardiogram in the treatment group were 94.07% and 87.29%

which were significantly higher than those of the control group of 82.76% and 76.72%（P<0.05）. After treatment

Conclusion Guanxin Danshen Dripping Pills may improve the efficacy in treating elderly unstable angina pectoris patients with qi deficiency and blood stasis syndrome. The mechanism may be related to the inhibition of platelet activation and the reduction of serum MMP-9

sICAM-1 and tPAI-1 levels.
