On Rational Application of Qingfei Paidu Decoction (清肺排毒汤) in Clinical Practice
Objective To summarize the adverse reactions of patients with coronavirus disease 2019(COVID-19) after using Qingfei Paidu Decoction(清肺排毒汤). Methods A total of 383 patients with COVID-19 who were hospitalized in the Western Hospital Region of Union Hospital
Tongji Medical College
Huazhong University of Science and Technology from February 18 to March 5
2020
were routinely given Arbidol hydrochloride tablets or lopinavir ritonavir tablets as well as Qingfei Paidu Decoction at the same time. Three days were a course of treatment
and a maximum of 3 courses were taken. The adverse reactions during the medication were observed. Patients with elevated serum transaminase were randomly divided into groups A and group B. Patients in group A stopped using Arbidol hydrochloride tablets or lopinavir ritonavir tablets and Qingfei Paidu Decoction. At the same time
patients in group B stopped taking Arbidol Hydrochloride tablets or lopinavir ritonavir tablets but continued to take Qingfei Paidu Decoction. The levels of aspartate aminotransferase(AST) and alanine aminotransferase(ALT) were compared between the 2 groups before treatment
after medication and after drug withdrawal. Results A total of 56 of the 338 patients(16.57%) had increased sweating
of which 2 cases(0.59%) had excessive sweating; 12 cases(3.55%) had elevated serum transaminase; 20 cases(5.92%) had epigastric pain; 16 cases(3.25%) had diarrhea symptoms; other adverse reactions such as nausea and vomiting
palpitations
insomnia
blood pressure and blood sugar changes
skin allergies had a low incidence and mild symptoms
and were relieved after stopping the drug. The 12 patients with elevated serum transaminase were randomized in the range of A and B groups
with 6 cases in each group. Compared with those before medication
AST and ALT in groups A and group B increased significantly after medication(P<0.01)
and the level returned to normal after medication withdrawal(P>0.05). There was no significant difference between the groups in AST
ALT in group A and B before treatment
after medication and withdraw of medication(P>0.05). Conclusion Patients with COVID-19 using Qingfei Paidu Decoction have a small amount of excessive sweating
epistomach pain
diarrhea
nausea and vomiting
heart palpitations
insomnia and other adverse reactions
elevated serum transaminase after medication might not be related to Qingfei Paidu Decoction.
