Influencing Factor of Hypertension Complicated with Stroke and the Construction of Preliminary Risk Prediction Model: A Retrospective Study of 3859 Cases
Objective To investigate the risk factors related to hypertension complicated with stroke
and to initially construct a risk prediction model for hypertension complicated with stroke. Methods Through Tianjin Community Health Service System(HIS)
data were retrospectively collected from 3859 hypertension patients who established the medical file in the Beichen District Community Health Service Center of Tianjin from January 1 st
2018 to December 31 st
2019 with complete data preservation. According to whether there was complication of stroke
the included patients were divided into stroke group(412 cases
10.68%) and non-stroke group(3447 cases
89.32%). Univariate and multivariate analysis were used to figure out the high-risk factors of hypertension complicated with stroke and further establish a predictive model. The Hosmer-Lemeshow goodness-of-fit test and receiver operating characteristic curve(ROC) were used to evaluate the predictive model. Results The univariate analysis showed that the white blood cell count
neutrophil percentage
lymphocyte percentage
fasting blood glucose
total bilirubin
blood creatinine
blood urea nitrogen
total cholesterol
triglycerides
low density lipoprotein cholesterol
high density lipoprotein cholesterol
age
sex
smoking history
meat and vegetable balanced and salty diet
meat-based salty diet
vegetarian-based salty diet
and qi deficiency were potential risk factors for hypertension complicated with stroke. The multivariate analysis showed that total cholesterol
low-density lipoprotein cholesterol
body mass index(BMI)
and meat-based salty diet were independent risk factors for hypertension complicated with stroke. The predictive model was established as follow: Logit(P)=1.580-0.610×total cholesterol+0.525×low-density lipoprotein cholesterol-0.075×BMI+2.064×meat-based salty food(1 represented yes
0 represented no)
with diagnostic sensitivity of 66.20%
specificity of 54.70%
and the agreement rate of 74.76%. Conclusion Total cholesterol
low-density lipoprotein cholesterol
BMI
and meat-based salty diet are considered to be the high-risk factors for hypertension complicated with stroke
and the risk prediction model established on these factors is found to be in good agreement with the clinical diagnosis
and can be used to quantitatively assess the risk of stroke in patients with hypertension.
