摘要

中医诊治疾病关键在于捕捉病因病机，故辨机论治是诊疗疾病的核心理论。针对辨机论治的源流、内涵和特点进行梳理和总结，探讨辨机论治和辨证论治的异同与互补关系。在临床实践的基础上，提出病机类型有初始病机、衍生病机、对证病机、共通病机、体质病机、时空病机、环境病机、兼夹病机、药毒病机、杂合病机、对症病机、局部病机，在临床应用时首先要根据不同疾病特点进行病机模式的归纳总结，灵活使用，分清主次，精准治疗。辨机论治有助于中医临证的精准化治疗，构建辨机论治的诊疗模式对提高临床疗效有重要意义。

Abstract

It is critical to figure out the mechanism in clinical practice， therefore， treatment on pathomechanism differentiation is considered as the core theory when diagnosing and treating diseases. This paper summarized the origin， connotation and characteristics of treatment based on pathomechanism differentiation， and explored the similarities， differences and complementary relationship between pathomechanism differentiation and syndrome differentiation. Based on the clinical practice， it is proposed that there are initial pathomechanism， derivative pathomechanism， syndrome pathomechanism， common pathomechanism， constitution pathomechanism， spatio-temporal pathomechanism， environmental pathomechanism， combined pathomechanism， drug toxin pathomechanism， mixed pathomechanism， symptomatic pathomechanism， and local pathomechanism. For clinical application， it is firstly necessary to differentiate the pathomechanism model according to the characteristics of the disease， and then use them flexibly， and distinguish the primary from the secondary ones， thereby treating the disease accurately. Treatment on pathomechanism differentiation is helpful to the precise treatment of traditional Chinese medicine， therefore， it is important to construct a diagnosis and treatment model based on pathomechanism differentiation so as to improve the clinical effectiveness.
