摘要

目的

探讨茵陈五苓散治疗阻塞性黄疸湿重于热证患者的临床疗效及安全性。

方法

采用随机对照试验设计方案，选取因胆总管结石造成的阻塞性黄疸湿重于热证患者76例，随机分为治疗组与对照组各38例。对照组患者在胆道术后给予西医常规治疗，治疗组术后第1天在常规治疗基础上加用茵陈五苓散方颗粒剂口服。两组均治疗7天。检测两组患者术前1天及术后1、5、7天肝功能指标，包括总胆红素（TBiL）、直接胆红素（DBiL）、碱性磷酸酶（ALP）、丙氨酸氨基转移酶（ALT）、γ-谷氨酰胺转肽酶（GGT），治疗前后检测两组患者血常规指标、心电图，并进行安全性评价。

结果

两组患者术后1天、术后5天、术后7天 TBiL、DBiL、ALP、ALT、GGT水平均较本组术前降低（

＜0.05）。与本组术后1天比较，两组患者术后5天、7天肝功能指标均改善（

＜0.05）；与本组术后5天比较，两组患者术后7天肝功能指标进一步改善（

＜0.05）。术后1天两组肝功能指标差异无统计学意义（

P＞

0.05），而术后5天、术后7天治疗组肝功能指标改善显著优于对照组（

＜0.01）。两组患者治疗前后血常规指标及心电图均未见明异常，均未出现不良反应。

结论

茵陈五苓散能够加快阻塞性黄疸湿重于热证患者胆道术后肝功能改善，降低肝损害的敏感指标，且安全性好。

Abstract

Objective

To explore the clinical effect and safety of

Yinchen Wuling Powder

（茵陈五苓散， YWP） in the treatment of obstructive jaundice patients with dampness predominating over heat pattern.

Methods

With randomized controlled trial design， a total of 76 obstructive jaundice patients with dampness predominating over heat pattern caused by choledocholithiasis were randomly divided into intervention group and control group with 38 cases in each group. After biliary surgery， the control group was given conventional western medicine therapy， and the intervention group received oral YWP additionally. The duration of treatment was 7 days in both groups. The liver function indicators including total bilirubin （TBiL）， direct bilirubin （DBiL）， alkaline phosphatase （ALP）， alanine amino transferase （ALT）， and γ-glutamine transpeptidase （GGT） levels were measured 1 day before surgery， and 1， 5， and 7 days after surgery. The blood markers and the electrocardiogram were assessed before and after treatment， and the safety outcomes were recorded.

Results

The levels of TBiL， DBiL， ALP， ALT and GGT in both groups significantly decreased 1， 5， and 7 days after surgery （

＜0.05）. Compared to those measured 1 day after surgery， the liver function indicators in both groups were improved 5 days and 7 days after surgery （

＜0.05）， and more improvements were seen 7 days after surgery （

＜0.05）. There was no significant difference between groups in liver function indicators 1 day after surgery （

＞0.05）， while more improvements were seen in the intervention group than in the control group 5 days and 7 days after surgery （

＜0.01）. There were no obvious abnormalities in blood markers and electrocardiogram in both groups， and no adverse reactions occurred.

Conclusion

YWP can accelerate the improvement of liver function， reduce liver damage-sensitive indicators， and is safe when treating obstructive jaundice patients with dampness predominating over heat pattern.
