摘要

目的

总结分析针刺捻转量效关系的研究现状，为临床与科研中针刺捻转刺激量的选择提供参考。

方法

检索中国知网（CNKI）、万方数据库（WF）、维普数据库（VIP）及PubMed数据库，收集2000年1月1日至2021年6月30日期间发表的针刺捻转量效关系的基础研究及临床研究文献，分析针刺捻转手法临床试验及动物实验中涉及的主要内容，对捻转手法的作用力、作用时间进行归纳，并分析刺激量的影响因素。

结果

共纳入59篇文献，研究对象涉及健康人、患者以及模型动物。临床试验文献显示，针刺捻转手法对多种疾病的治疗效果有影响，不同捻转刺激量的针刺效应存在差别，捻转的强度、方向、频率均对疗效产生影响。动物实验结果显示，针刺捻转手法可通过不同途径介导调节机体的生命活动或干预病理过程。11篇文献比较了不同捻转刺激量的量效差异，采用的幅度范围为0~720°，频率为0~480次/min，操作时间为10 s~5 min，随着参数值的增大，刺激的强度越大，大部分研究发现中小刺激量的针刺疗效优于重刺激，但刺激强度大小的描述缺乏统一认识。

结论

目前针刺捻转刺激量的研究缺乏不同参数的对照，难以比较最佳刺激效应，后续研究应进一步规范针刺手法研究的试验设计，综合分析量效规律，选择适当的刺激量，提高针刺疗效。

Abstract

Objective

To analyze the research literature about dose-response relationship of acupuncture twirling method， thereby providing reference for the selection of suitable stimulation amount when performing acupuncture twirling in clinical practice and scientific research.

Methods

The databases including CNKI， Wanfang， VIP and PubMed were searched for the laboratory and clinical research papers about dose-response relationship of acupuncture twirling method published from January 1st， 2000 to June 30th， 2021. The main contents related to acupuncture twirling method in the clinical trials and basic experiments were collected to summarize its force and duration， and to analyze the influencing factors of stimulation amount.

Results

A total of 59 articles were included involving healthy people， disease patients and model animals. Clinical studies showed that acupuncture twisting method has certain therapeutic effect on various diseases， and the effect varied between different stimulation amounts including twirling intensity， direction and frequency. The results of animal experiments showed that acupuncture twisting method can mediate and regulate the body's life activities or intervene in the pathological processes through different pathways. Eleven papers compared the effects between different stimulation amount of twirling， which was at a range of 0 to 720°， with a frequency of 0 to 480 times per minute， and for 10 s to 5 min. The results showed that the stimulation amount increased as the parameter values， and in most studies， small-moderate amount of stimulation was correlated to a better effect than large amount of stimulation. However， the definition of small-moderate and large amount of the stimulation was not unified.

Conclusion

Currently， the research on the amount of acupuncture twisting stimulation lacks the comparison of different parameters， thus it is difficult to compare the optimal stimulation effect. Further research with standardized design should be conducted by comprehensively analyzing the dose-response relationship and selecting an appropriate stimulation amount， so as to improve the acupuncture effect.
