Sepsis with Gastrointestinal Dysfunction Treated by Adjuvant Enema Administration of Tiaochang Formula (调肠方) in 30 Cases
To explore the clinical effect of adjuvant enema administration of
Tiaochang Formula
(调肠方) in the treatment of sepsis with gastrointestinal dysfunction.
Methods
2
Sixty sepsis patients with gastrointestinal dysfunction were randomly divided into treatment group and control group, with 30 cases in each group. The control group received conventional western medicine, and the treatment group was given enema administration of
Tiaochang Formula
in addition. The treatment course of both groups was seven days. The acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) score, the intestinal mucosal barrier function indicators including serum diamine oxidase (DAO), vasoactive intestinal peptide (VIP) and D-lactic acid, and the inflammatory indicators including tumor necrosis factor alpha (TNF-α) and interleukin 6 (IL-6) were assessed before and after treatment. The effect regarding traditional Chinese medicine (TCM) syndrome score and gastrointestinal function of the two groups were calculated, and the incidence of multiple organ dysfunction syndrome (MODS) and 28-d mortality were assessed during treatment.
Result
2
No patient was clinically cured in both groups. In the treatment group, 10 cases were markedly effective, 11 cases were effective and 9 cases were ineffective, the total effective rate was 70.00%. In the control group, the corresponding cases were 6, 6 and 18, and the total effective rate was 40.00%, significantly lower than that in the treatment group (
P
<0.05). After treatment, the APACHE Ⅱ score, the intestinal mucosal barrier function indicators (DAO, VIP, D-lactic acid) and the inflammatory indicators (TNF-α, IL-6) significantly decreased in both groups, and were much lower inthe treatment group than in the control group (
P
<0.05). The incidence of MODS in the treatment group was 16.70%, significantly lower than 40.00% in the control group (
P
<0.05). There was no significant difference in 28-d mortality between the treatment group (13.3%) and the control group (30.0%,
P
>0.05).
Conclusion
2
Adjuvant enema administration of
Tiaochang Formula
can improve gastrointestinal function, reduce inflammatory response, reduce the incidence of MODS and improve the prognosis of sepsis patients with gastrointestinal dysfunction.
