摘要

初步建立冠状动脉血运重建后双心疾病（心理心脏病）痰瘀互结、忧郁气结证的诊断及疗效评价量表，为衡量中医药干预双心疾病的疗效提供依据。检索中国知网、中国科技期刊数据库、万方数据知识服务平台中关于冠状动脉血运重建后双心疾病的相关文献，并参考相关行业标准及共识，共纳入文献32篇，参考行业标准及共识9篇，提取其中痰浊证、血瘀证、气郁证及其复合证型所涉及的症状体征及舌象、脉象等要素，形成的初步量表涵盖痰、瘀、气、心理4个维度共42个条目。对形成的初步量表进行德尔菲专家调查，邀请专家39位，发放两轮问卷（有效率均为100%），针对各条目与痰瘀互结、忧郁气结证冠脉血运重建后双心疾病的相关性进行赋分，一轮问卷根据条目重要性结果删除3项条目并补充11项条目，内容效度系数0.94，内容信度系数0.96；二轮问卷删除8项条目后内容效度系数为0.95，信度系数为0.975，各条目变异系数＜0.25，以二轮问卷为最终结果，专家熟悉程度总体得分0.828，专家权威系数0.893。最终量表包含42个条目，分为重要、主要、一般三类，分别赋分3分、2分、1分，各条目得分总和为量表最终得分。此量表在一定区域内具有良好的有效性、可靠性、实用性和权威性，可用于冠状动脉血运重建后双心疾病痰瘀互结、忧郁气结证的诊断及疗效评价。

Abstract

This paper aimed to preliminarily develop the diagnosis and effect evaluation scale of psycho-cardiological diseases with the syndrome of binding of phlegm and stasis， and

constraint and knot after coronary revascularization， so as to provide a basis for measuring the therapeutic effect of TCM in the treatment of psycho-cardiological diseases. The databases including CNKI， VIP， and Wanfang Database were searched for relevant literature on psycho-cardiological diseases after coronary revascularization， and the relevant industry standards and consensus were referred to. Finally， 32 literature and 9 industry standards and consensus were included. The symptoms， tongue manifestations， pulse manifestations and other elements of phlegm turbidity syndrome， blood stasis syndrome，

constraint syndrome and the composite syndrome were extracted. And the preliminary scale was formed which was composed of four domains including phlegm， stasis，

and psychology and 42 items. A two-round Delphi expert survey （response rate： 100%） of 39 experts was conducted to evaluate the correlation of each item and the psycho-cardiological diseases with the syndrome of binding of phlegm and stasis， and

constraint and knot after coronary revascularization. In the first round survey， three items were deleted and 11 items were added regarding the importance results， with a content validity coefficient of 0.94 and a content reliability coefficient of 0.96. In the second round survey， 8 items were deleted with a validity coefficient of 0.95 and reliability coefficient of 0.975， and the coefficient of variation was smaller than 0.25. The familiarity with items was 0.828， and the authoritative coefficient （Cr） was 0.893. The final scale was composed of 42 items， which can be rated as important， main， and general with 3 points， 2 points， and 1 point respectively. The sum of these items was the total score. This scale has good validity， reliability， practicability and authority in a certain area， and can be used for the diagnosis and effect evaluation of psycho-cardiological diseases with the syndrome of binding of phlegm and stasis， and

constraint and knot after coronary revascularization.
