The Preliminary Development of Syndrome Therapeutic Effect Evaluation Scale for Stable Chronic Obstructive Pulmonary Disease based on Clinical Survey
To preliminarily form a syndrome therapeutic effect evaluation scale for stable chronic obstructive pulmonary disease (COPD) based on clinical survey.
Methods
2
Clinical survey among the stable COPD patients who met the inclusion criteria was conducted in 13 traditional Chinese medicine/ integrated medicine hospitals to collect data on basic information of patients, disease assessment, and therapeutic evaluation of nine basic syndromes (including cold rheum syndrome, phlegm-damp syndrome, blood stasis syndrome, lung
qi
deficiency syndrome, spleen
qi
deficiency syndrome, kidney
qi
deficiency syndrome, lung
yin
deficiency syndrome, kidney
yin
deficiency syndrome, kidney
yang
deficiency syndrome) and corresponding 78 items. The discrimination parameter (a) and difficulty parameter (b) based on item response theory (IRT) model were used to analyze items, and a ≥0.3 with b = [-4, 4] were set as the chosen criteria of items. The item characteristic curve (ICC) and item information curve (IIC) were used to visualize the items. The items were selected, and the expression forms of items were optimized. Thus, syndrome therapeutic evaluation scale of stable COPD was preliminarily developed.
Results
2
Of the 580 questionnaires, 576 were responded and 568 were valid with a validity of 98.6%. A total of 2394 cases were investigated, including 73 cases of cold rheum syndrome, 397 cases of phlegm-damp syndrome, 200 cases of blood stasis syndrome, 855 cases of lung
qi
deficiency syndrome, 265 cases of spleen deficiency syndrome, 262 cases of kidney
qi
deficiency syndrome, 172 cases of lung
yin
deficiency syndrome, 81 cases of kidney
yin
deficiency syndrome, and 89 cases of kidney
yang
deficiency syndrome. According to discrimination parameter (a) and difficulty parameter (b) based on IRT model, as well as the ICC and IIC results, certain items were deleted including 6 items of cold rheum syndrome, 11 items of phlegm-damp syndrome, 8 items of blood stasis syndrome, 7 items oflung
qi
deficiency syndrome, 12 items of spleen
qi
deficiency syndrome, 14 items of kidney
qi
deficiency syndrome, 5 items of lung
yin
deficiency syndrome, 9 items of kidney
yin
deficiency syndrome, and 7 items of kidney
yang
deficiency syndrome. The items and its expression were further optimized, and then the syndrome therapeutic evaluation scale of 9 basic syndromes and 46 items was developed, involving 9 items of cold rheum syndrome, 12 items of phlegm-damp syndrome, 6 items of blood stasis syndrome, 10 items of lung
qi
deficiency syndrome,14 items of spleen
qi
deficiency syndrome, 13 items of kidney
qi
deficiency syndrome, 9 items of lung
yin
deficiency syndrome, 8 items of kidney
yin
deficiency syndrome, and 9 items of kidney
yang
deficiency syndrome.
Conclusion
2
The syndrome therapeutic effect evaluation scale for stable COPD has been preliminarily developed based on clinical survey and IRT model, which includes 9 basic syndromes and 46 items, and could be further explored by determining the item weight.
