Effect and Safety Evaluation of Different Frequency of Fire Needling on Knee Osteoarthritis with Cold-dampness Obstruction Syndrome:a Randomize-Controlled Clinical Trial
To evaluate the clinical effect of different frequency of fire needling in the treatment of knee osteoarthritis (KOA) with cold and dampness obstruction syndrome.
Methods
2
A total of 227 KOA patients with cold and dampness obstruction syndrome were randomly divided into control group (80 cases), low frequency treatment group (75 cases) and high frequency treatment group (72 cases). The low and high frequency treatment group were given fire needling at Xuehai (SP10), Liangqiu (ST34), Neixiyan (EX-LE4), Waixiyan (EX-LE5),Yanglingquan (GB34) and Ashi points of the affected limb, once every five days or once every three days, respectively. The control group was given oral celecoxib capsules, once a day, 0.2 g each time. All the groups were treated for 30 days. The changes of the knee joint range of motion (ROM), circumference, and effusion, the synovial thickness, serum inflammatory factors, Western Ontario and McMaster University Osteoarthritis Index (WOMAC), and visual analogue scale (VAS) were assessed and compared before and after treatment, and the safety was evaluated.
Results
2
Before the treatment, there were no significant differences in the knee joint ROM, circumference, and effusion, synovial thickness, serum inflammatory factors, the WOMAC score and the VAS score among all groups (
P
>0.05). After treatment, knee joint ROM, circumference and effusion volume, the synovial thickness, serum inflammatory factors levels, the WOMAC and the VAS score in both low and high frequency treatment groups were significantly improved (
P
<0.05), while those in the control group were not significantly changed (
P
>0.05). The improvement of knee joint range of motion, circumference, and effusion volume, the synovial thickness, serum inflammatory factors, the WOMAC and the VAS scores in the low and high frequency treatment groups were better than those in the control group (
P
<0.05), and were superior in the high frequency treatment group than the low frequency treatment group (
P
<0.05) except for the joint circumference (
P
>0.05). There were no obvious abnormalities in safety indicators in each group.
Conclusion
2
Compared with the low frequency fire needle treatment, Fire needling at high frequency is better than low frequency in reducing serum inflammatory factors levels, reducing the knee effusion volume, impro-ving the synovial thickness, increasing the knee joint ROM, and improving the pain, stiffness and mobility disorders in patients with KOA.
