摘要

营卫别出两行、交感贯通、气血互渗、抵御外邪的生理功能依赖于肺络细小而微、网状分布、末端相通、布散皮毛的特殊结构。营卫不和，肺络结构因而改变，与肺结节“结癌转化”的恶性渐进过程密切相关。基于此，认为肺结节病机演变初始于“卫阳不煦，寒湿停聚，肺络失荣”，形成于“营卫不从，肺络受损，痰瘀胶结”，癌变于“营泣卫除，肺络虚损，癌毒内生”。以营卫-肺络相失为纲，结合影像学特征，初始期以生活方式干预及非药物治疗为主，形成期宜消痰祛瘀、通达营卫、畅通肺络，癌变期需解毒散结、扶正固本、通补肺络，为中医防治肺结节、干预结癌转化提供思路。

and blood， and can resist external external pathogens. All these physiologcial functions are depended on the special structure of lung collaterals， which are tiny and thready， distributed in network with end communication， and scattered widely in the skin and body hair. Disharmony between

and

wei

leads to change in the structure of pulmonary collaterals， which is closely related to the malignant progressive process of nodules-cancer transformation. Therefore， we believe that the dynamic evolution of pulmonary nodules is initiated from “

wei yang

failing to warm， cold-dampness accumulation， and deficiency of lung collaterals”， gradually formed due to “disorder of

ying

wei qi

， damage of lung collaterals， and intermingled phlegm and blood stasis”， and deteriorated if there is “depletion of

Ying

wei

and lung collaterals， and internal generation of cancer toxin”. On the basis of incongruence between

ying

wei

and lung collaterals as well as the imaging features， lifestyle adjustment and non-pharmacological treatment are suggested in the initial stage. In the formation stage， it is appropriate to disperse phlegm and dispel stasis， unblock and harmonize

ying

wei

， and dredge the lung collaterals. In the deteriorated stage， it is necessary to resolve toxins and dissipate masses， reinforce healthy

and consolidate the root， and unblock and replenish the lung collaterals. All mentioned above may provide ideas for using traditional Chinese medicine to prevent and treat lung nodules， and to intervene in the transformation of nodules-cancer.
