Objective

To observe the effect of Wei's two-step seven manipulations on spinal-pelvic three-dimensional imaging parameters in patients with lumbar disc herniation （LDH）.

Methods

A total of 72 LDH patients were randomly divided into two groups； 36 cases in the manipulation group were treated with Wei's two-step seven manipulations，while 36 cases in the control group were treated with conventional tui na，and both groups were treated three times a week for 4 consecutive weeks. Spinal-pelvic three-dimensional imaging was performed before and after treatment， and the parameters included lateral pelvic tilt （LPT）， lumbar lordotic angle （LL）， pelvic incidence （PI）， pelvic tilt （PT）， sacral slope （SS）， and sagittal vertical axis （SVA）； vertebral spatial position indexes included L5 axial rotation， L5 vector displacement （x）， L5 vector displacement （y）， L4 axial rotation， L4 vector displacement （x）， and L4 vector displacement （y）. Pain visual analogue score （VAS） and Oswestry disability index questionnaire （ODI） score before and after 2- and 4-week treatment were measured.

Finally， 35 cases in the manipulation group and 34 cases in the control group were included in the analysis. After treatment， LPT and SVA values decreased in both groups， while LL value increased in the manipulation group （

＜0.001）， with more change in the manipulation group in terms of LPT and SVA values （

＜0.001）. Regarding the L4 and L5 axial rotation， L5 vector displacement （x）， and L5 vector displacement （y）， much more improvements were found in the manipulation group than in the control group （

＜0.01）. Compared to those before treatment， the VAS score and ODI score significantly decreased in both groups when measured after 2- and 4-week treatment （

＜0.001）. After 4 weeks of treatment， the VAS score and ODI index of patients in the manipulation group were significantly lower than those in the control group （

Wei's two-step seven manipulations can effectively promote the recovery of spine-pelvis biomechanical balance than conventional tuina， which may be one of the reasons why it is superior to conventional tuina in alleviating pain and restoring dysfunction.
