Scoping Review of Clinical Trials on Traditional Chinese Medicine in Treating Hypertension from 2010-2022
To review the current status of clinical trials on hypertension treated with traditional Chinese medicine (TCM) in recent years.
Methods
2
The clinical trials on hypertension treated with TCM therapy in Chinese National Knowledge Infrastructure (CNKI), WIP database, Wanfang data platform, and PUBMED database was retrieved from January 1, 2010 to October 1, 2022. Relevant information including research design, research content, administration of medicinals, TCM therapy, sample quantity, research cycle, efficacy indicators were extracted and statistically analyzed. The quality of the included studies was evaluated using the
Cochrane Handbook for Systematic Reviews of Interventions
and MINOR items.
Results
2
A total of 192 studies were included with 185 randomize controlled trials (RCT), and 7 non-RCTs. Among the 185 RCTs, the high risk frequency of the generation of random sequences, hiding random scheme, blind method, and other bias assessment results was 87 (47.03%), 167 (90.27%), 169 (91.35%), 5 (2.70%) respectively. The minimum score was 6 points (2 items) and the maximum score was 10 points (1 item) in the 7 non-RCTs. Research content focused on the efficacy evaluation of the Chinese and Western combination protocols (76.04%, 146/192), self-prescription being the maxmum in Chinese medicinals (66.15%, 127/192), followed by combined
Tianma Gouteng
Beverage (天麻钩藤饮). The mainly used wester medicines are calcium ion antagonists (47.40%, 91/192), angiotensin converting enzyme inhibitor/angiotensin receptor blockers (38.02%, 73/192). TCM therapy includes calming the liver, supplementing kidney, fortifying the spleen, eliminating phlegm, rectifying
qi
, regulating blood, subduing fire, unblocking the collaterals, etc. Of which, the treatment methods of calming the liver, soothing the liver, tranquilizing the liver, and clearing liver heat were the most widely used (43.23%, 83/192). The research samples ranged from 30 to 852 cases, with the mode of 60 cases and the median of 75 cases. The research cycle was from 2 to 52 weeks with 8 weeks (30.73%, 59/192) and 4 weeks (29.69%, 57/192)being commonly used. Except from clinical blood pressure (100%), other efficacy indicators were 24 h dynamic blood pressure (25.52%, 49/192), renal function indicators (13.02%, 25/192), and arteriosclerosis indicators (9.38%, 18/192).
Conclusion
2
The clinical trials on hypertension treated with TCM therapy has a variety of methods and abundant prescriptions, which can confirm that the combination of traditional Chinese and western medicine has a clear advantage. However, the overall research still has problems such as unclear research directions and purposes, short research cycles, and high risk of bias.
