Correlation between Chinese Medicine Constitution and Coronavirus Diseases 2019 (COVID-19): A Systematic Review and Meta-analysis
To investigate the distribution of traditional Chinese medicine (TCM) constitution among the patients with coronavirus diseases 2019 (COVID-19), and the correlation between the TCM constitution and the occurrence and prognosis of COVID-19.
Methods
2
The China National Knowledge Infrastructure (CNKI), Chongqing VIP, Wanfang Data, PubMed, and Web of Science databases were searched from January 1st, 2020 to July 24th, 2022. Clinical studies evaluating the relationship between body constitution of TCM and COVID-19 were included. The primary outcome included the distribution of 9 TCM constitutional types among the COVID-19 population, risk ratio and odds ratio between constitution and COVID-19. Effect size was expressed as proportion or odds ratio with 95% confidence interval (
CI
). Methodological quality of included studies was evaluated by the international evaluation criteria for observational studies.
Results
2
Ten cross-sectional studies involving 1106 cases of COVID-19 were included. There was no control group of general population in all studies. Phlegm-dampness,
qi
-deficiency, and balanced constitution accounted for 26% (95%
CI
: 17%-38%), 20% (95%
CI
: 13%-29%), and 12% (95%
CI
: 6%-22%) of COVID-19 patients, respectively. Subgroup analysis showed that there were some differences in the constitution of patients with different virus strains, disease grades and age. Meta-analysis of two studies showed that there was no statistical difference in duration of nucleic acid turning negative between phlegm-dampness type and balanced constitution (
SMD
=0.48, 95%
CI
: -0.52-1.49,
P
= 0.35). One study reported that the hospitalization time was 11.7±4.4 days in patients with the balanced constitution, which was shorter than the 19.7±8.5 days in those with yin deficiency constitution (
P
<0.05). The methodological items of each study was generally poor in reporting, and suggested a high risk of bias among the studies.
Conclusion
2
Phlegm-dampness and
qi
deficiency are the main constitutional types of patients with COVID-19, and may be the susceptible population of COVID-19. There is few evidence about the relationship between constitution and prognosis of COVID-19. In the future, cohort study on the correlation between TCM constitution and prognosis could be carried out considering the factors of regions, populations, virus strains and infection times, in order to provide reference for the prevention and treatment of COVID-19.
