Treatment of Atopic Dermatitis Based on Treatise on the Spleen and Stomach (《脾胃论》)增强出版
(《脾胃论》), LI Gao discussed the relationship between spleen earth, heart fire and lung metal, and proposed that spleen earth insufficiency can lead to the heart fire attack as well as lung metal insufficiency, that is, earth deficiency causes heart attack and brings metal deficiency. Spleen deficiency is the key factor in the formation of atopic dermatitis, and in the development of this disease, the pathogenesis often evolves from single spleen deficiency to spleen-lung insufficiency and insecurity of exterior, then earth deficiency with fire attack and dampness-heat invasion. According to LI Gao's understanding of the relationship between spleen earth, heart fire and lung metal, as well as the disease characteristics of atopic dermatitis, sweet-warm medicinals such as Renshen (
Radix et Rhizoma Ginseng
) and Gancao (
Radix et Rhizoma Glycyrrhizae
) are suggested to nourish earth and drain fire, and wind medicinals such as Chaihu (
Radix Bupleuri
) and Qianghuo (
Radix et Rhizoma Notopterygii
) are used to ascend and disperse
yang qi
, diffuse and unblock
qi
and fluid, and overcome dampness, trying to treat the root. In addition, cool medicinals such as Huanglian (
Rhizoma Coptidis
) and Huangqin (
Radix Scutellariae
) are suggested to drain heart fire, and for obvious rash or itching, more and large amount of cool medicinals are necessary to control the fire, all of which is to treat the branch. In clinical practice, the proportion of sweet-warm medicinals, wind medicinals, and cool medicinals should be determined according to the severity of spleen deficiency, lung deficiency, and heart fire.
