Survey on the Differences in Disease Activity between Kidney Deficiency Obstruction Syndrome and Damp-heat Obstruction Syndrome of Ankylosing Spondylitis Patients
To compare the characteristics of disease activity between kidney deficiency obstruction syndrome and damp-heat obstruction syndrome in patients with ankylosing spondylitis (AS).
Methods
2
A survey was conducted in seven centers nationwide to collect data on AS patients with kidney deficiency obstruction syndrome and damp-heat obstruction syndrome using
Ankylosing Spondylitis Syndrome Information Collection Scale
. The AS di-sease activity score with C-reactive protein (ASDAS-CRP), AS disease activity score-erythrocyte sedimentation rate (ASDAS-ESR), Bath ankylosing spondylitis disease activity index (BASDAI) score, patient global assessment (PGA) score, single symptom (low back pain, peripheral joint pain/swelling, duration of morning stiffness, degree of morning stiffness, tenderness/pressing pain and fatigue) scores, C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) levels were assessed in the two groups. The disease activity of patients in the two groups was evaluated according to ASDAS-CRP.
Results
2
Totally, 684 AS patients with kidney deficiency obstruction syndrome and 558 AS patients with damp-heat obstruction syndrome were included. There was no significant difference in PGA score, CRP and ESR levels between the two groups (
P
>0.05). In terms of the single symptom, the scores of low back pain, peripheral joint pain/swelling, duration of morning stiffness and tenderness/pressing pain were higher in damp-heat obstruction syndrome patients than in kidney deficiency obstruction syndrome patients (
P
<0.05 or
P
<0.01). The ASDAS-CRP, ASDAS-ESR and BASDAI scores were much higher in patients with damp-heat obstruction syndrome (
P
<0.01). There were 79 (11.55%) clinical remission cases, 178 (26.02%) low disease activity cases, 338 (49.42%) high disease activity cases, and 89 (13.01%) very high disease activity cases in patients with kidney deficiency obstruction syndrome, while the proportion was 53 (9.50%), 126 (22.58%), 295 (52.87%) and 84 (15.05%) in patients with damp-heat obstruction syndrome, with significantly differences in the distribution of disease activity levels between groups (
P
<0.05).
Conclusion
2
AS patients with damp-heat obstruction syndrome have higher disease activity and higher proportion of high and very high disease activity status than those with kidney deficiency obstruction syndrome.
