Treatment of Low γ-Glutamyl Transpeptidase Type of Infantile Cholestatic Liver Disease with Liver Colla-terals Obstruction and Damp-heat Internal Accumulation Syndrome by Tongluo Lidan Decoction (通络利胆汤):72 Cases Clinical Observation
(通络利胆汤) in the treatment of low γ-glutamyl transpeptidase (GGT) type of infantile cholestatic liver disease (CLD).
Methods
2
A total of 144 low GGT type of CLD infants who were admitted to the outpatient clinic and ward of the department of traditional Chinese medicine of Beijing Children's Hospital from June 1st, 2019 to October 31st, 2022 and were diagnosed as liver collaterals stagnation and internal damp-heat accumulation syndromes were included, and they were divided into 72 cases each in the treatment and control groups according to the random number table. The control group was treated with oral ursodeoxycholic acid capsules, glucuronide tablets and fat-soluble vitamins, while the treatment group was additionally given oral Chinese medicine
Tongluo Lidan Formula
(通络利胆方), both for 12 weeks. The remission rate of cholestasis after 12-week treatment was assessed and compared between groups. Effect regarding the traditional Chinese medicine (TCM) syndromes after 2-, 4-, 8- and 12-week treatment were assessed, as well as the recovery of liver function indicators including total bilirubin (TBIL), direct bilirubin (DBIL), GGT, total bile acid (TBA), alkaline phosphatase (ALP), glutamate aminotransferase (ALT), and aspartate aminotransferase (AST). The changes in blood hyaluronic acid (HA) after 4- and 12-week treatment were compared between groups. The incidence of liver cirrhosis decompensation was compared after 6-month follow up.

Finally, 67 cases in the treatment group and 62 cases in the control group completed the trial, and intention-to-treat (ITT) analysis was used for the 15 drop-outs. After treatment, the remission rate of cholestasis was higher in the treatment group (23/72, 31.94%) than in the control group (11/72, 15.28%,
P
<0.05). For the effect regarding the TCM syndromes, the total effective rate was higher in the treatment group(57/72,79.17%) than in the control group (43/72, 59.72%,
P
<0.05). In the treatment group, the levels of TBIL, DBIL, AST and ALT decreased after 4-, 8- and 12-week treatment (
P
<0.05 or
P
<0.01), and the ALP level decreased after 12-week treatment (
P
<0.01). In the control group, the TBIL, ALT, and ALP levels decreased after 8- and 12-week treatment (
P
<0.05 or
P
<0.01), and DBIL and AST levels decreased after 4-, 8- and 12-week treatment (
P
<0.05 or
P
<0.01), while TBA level increased after 2- and 8-week treatment (
P
<0.05). Compared between groups, there were lower TBIL level after 4- and 12-week treatment (
P
<0.05) and lower DBIL level after 4-, 8- and 12-week treatment (
P
<0.05) in the treatment group than in the control group; the treatment group had higher AST level after 4- and 8-week treatment (
P
<0.05 or
P
<0.01) and lower ALT level after 4-week treatment (
P
<0.05) than the control group. The ALP level in the control group was significantly lower than that in the treatment group after 8-week treatment (
P
<0.01), while it was higher in the control group after 12-week treatment (
P
<0.05). The TBA level was significantly lower in the treatment group than in the control group after 4- and 8-week treatment (
P
<0.05 or
P
<0.01), while it was lower in the control group than in the treatment group after 12-week treatment (
P
<0.05). The HA level in the treatment group decreased after 12-week treatment (
P
<0.01). Fifty-five cases in the treatment group and 42 cases in the control group were followed up for 6 months; two cases of liver cirrhosis decompensation occurred in the treatment group as well as one case in the control group with no significant difference (
P
>0.05)).
Conclusion
2
Tongluo Lidan Decoction
in treatment of low γ-GT type of infantile CLD can effectively improve cholestasis, relieve clinical symptoms, slow down the process of liver fibrosis and promote remission.
