摘要

目的

观察针刺结合刺血拔罐治疗慢性自发性荨麻疹（CSU）的临床疗效并探讨其可能的效应机制。

方法

将70例CSU患者随机分为氯雷他定组和针刺+刺血组，每组35例。氯雷他定组予氯雷他定片口服，每次10 mg，每晚１次；针刺+刺血组予中脘、关元、天枢、足三里、三阴交、血海、曲池、合谷、太冲、百会、神庭穴针刺，每日1次，配合大椎、膈俞刺血拔罐，隔日1次。两组均治疗4周。分别于治疗前后评价两组患者7日荨麻疹活动度评分（UAS7评分），并检测血清免疫球蛋白E（IgE）、白细胞介素4（IL-4）、白细胞介素5（IL-5）、嗜酸性粒细胞阳离子蛋白（ECP）和血浆组织因子（TF）、活化的凝血因子Ⅶ（FⅦa）、凝血酶原片段1+2（F1+2）、D-二聚体（D-D）、补体成分5a（C5a）含量。

结果

最终共65例患者纳入结果分析，其中氯雷他定组32例、针刺+刺血组33例。两组患者治疗前UAS7评分，血清IgE、IL-4、IL-5、ECP含量，血浆TF、FⅦa、F1+2、D-D、C5a含量差异均无统计学意义（

＞0.05）。与本组治疗前比较，两组治疗后UAS7评分，血清IgE、IL-4、IL-5及血浆TF、FⅦa、F1+2、D-D、C5a含量均显著降低（

＜0.01）。治疗后两组患者UAS7评分及血清ECP、IgE、IL-4、IL-5含量差异均无统计学意义（

＞0.05），针刺+刺血组血浆TF、FⅦa、F1+2、D-D、C5a含量低于氯雷他定组（

＜0.05或

＜0.01）。

结论

针刺结合刺血拔罐可有效改善CSU患者皮损症状，降低其体内炎性因子水平，其效应机制可能通过调控患者凝血-补体-肥大细胞活化轴，进而抑制肥大细胞脱颗粒反应而实现。

Abstract

Objective

To observe the clinical efficacy of acupuncture combined with bloodletting and cupping in the treatment of chronic spontaneous urticaria（CSU） and to explore its potential mechanisms of action.

Methods

Seventy CSU patients were randomly divided into loratadine group and acupuncture + bloodletting group， with 35 patients in each group. The loratadine group received oral loratadine tablets， 10 mg once daily in the evening. The acupuncture + bloodletting group received acupuncture at Zhongwan （CV 12）， Guanyuan （CV 4）， Tianshu （ST 25）， Zusanli （ST 36）， Sanyinjiao （SP 6）， Xuehai （SP 10）， Quchi （LI 11）， Hegu （LI 4）， Taichong （LR 3）， Baihui （GV 20）， and Shenting （GV 24）， once daily，along with bloodletting and cupping at Dazhui （GV 14） and Geshu （BL 17）， every other day. Both groups were treated for 4 weeks. The 7-day urticaria activity score（UAS7） was assessed before and after the treatment， and levels of serum immunoglobulin E （IgE）， interleukin-4 （IL-4）， interleukin-5 （IL-5）， eosinophil cationic protein （ECP）， plasma tissue factor （TF）， activated factor Ⅶ （FⅦa）， prothrombin fragment 1+2 （F1+2）， D-dimer （D-D） and complement component 5a （C5a） were detected.

Results

A total of 65 patients were included in the final analysis， 32 in the loratadine group and 33 in the acupuncture + bloodletting group. Before treatment， there was no significant difference in UAS7 score， serum IgE， IL-4， IL-5， ECP levels， or plasma TF， FⅦa， F1+2， D-D， C5a levels between groups （

＞ 0.05）. After treatment， both groups showed significant reductions in UAS7 score， serum Ig

E， IL-4， IL-5， and plasma TF， FⅦa， F1+2， D-D， and C5a levels compared to those before treatment （

＜0.01）. However， after treatment， there was no significant difference in UAS7 score and serum ECP， IgE， IL-4， IL-5 levels between groups （

＞0.05）. The acupuncture + bloodletting group showed lower plasma TF， FⅦa， F1+2， D-D and C5a levels compared to the loratadine group （

＜0.05 or

＜0.01）.

Conclusion

Acupuncture combined with bloodletting and cupping can effectively improve the skin symptoms of CSU patients and reduce the levels of inflammatory factors. The potential mechanism of action may involve the regulation of the coagulation-complement-mast cell activation axis， thereby inhibiting mast cell degranulation.
