Latent Class and Latent Structure Analysis of Traditional Chinese Medicine Syndrome Distribution in 100 Cases of Children After Hematopoietic Stem Cell Transplantation增强出版
To explore the characteristics of traditional Chinese medicine (TCM) syndromes in children after hematopoietic stem cell transplantation (HSCT) and provide a reference for clinical syndrome differentiation and treatment.
Methods
2
A total of 100 pediatric patients after HSCT were enrolled. Basic information and TCM four-diagnosis data were collected. Latent class analysis (LCA) and latent structure analysis (LSA) were performed using Mplus 8.3 and Lantern 5.0, respectively, to summarize the TCM syndrome characteristics in children after HSCT.
Results
2
A total of 69 symptoms and signs were included. The five most frequent symptoms/signs were pale-red tongue body (77 cases), white tongue coating (66 cases), thready pulse (64 cases), poor appetite (62 cases), and fatigue (59 cases). Fifty-six TCM symptoms/signs with frequency >5 were subjected to LCA, resulting in a four-class latent model. Clinical interpretation identified the main syndromes as dual deficiency of
qi
and
yin
, dual deficiency of the spleen and kidney, lung-spleen
qi
deficiency, and
yin
deficiency with dampness. LSA of all 69 TCM symptoms/signs yielded nine latent variables. Based on TCM theory and clinical practice, these latent variables were clustered into five syndromes, lung-spleen
qi
deficiency, dual deficiency of
qi
and
yin
, spleen deficiency with dampness encumbrance, dual deficiency of the spleen and kidney, and
yin
deficiency with internal heat, partially accompanied by blood stasis.
Conclusion
2
Common TCM syndromes in children after HSCT include dual defic
iency of
qi
and
yin
, spleen deficiency with dampness encumbrance, lung-spleen
qi
deficiency, dual deficiency of the spleen and kidney,
yin
deficiency with internal heat,
yin
deficiency with dampness, and partial syndromes accompanied by blood stasis. The main affected organs are the spleen, lung, and kidney; deficiency is the predominant nature of disease, often accompanied by excess.
