Syndrome Differentiation and Treatment of Portal Vein Thrombosis Based on the Principle of "Presence of Cause Without Demise"
(《黄帝内经》) principle of "presence of cause without demise", a tiered syndrome differentiation and treatment approach for portal vein thrombosis is proposed. Addressing the "cause" of stubborn blood stasis toxin, treatment is divided into three stages, breaking blood stasis, eliminating stasis, and harmonizing blood. The breaking stage requires forceful intervention to disperse and dislodge congealed blood; the activating stage focuses on clearing turbidity, resolving stasis, and facilitating movement while reducing the dosage of strong medicines; the harmonizing stage emphasizes regulating
qi
and blood, nourishing to promote circulation, and treating both manifestation and root
cause to prevent recurrence. Throughout treatment, the principle of "stopping after reducing the majority" is applied, with dynamic adjustment of the balance between attack and tonification, considering concomitant syndromes such as phlegm, heat, cold, and deficiency. Emphasis is placed on strengthening the body while eliminating pathogens, integrating attack and tonification to enhance efficacy and reduce toxicity, embodying the traditional Chinese medicine (TCM) holistic perspective and dynamic balance of eliminating pathogens without harming the vital
qi
and strengthening the body without retaining stasis. This provides a therapeutic approach for portal vein thrombosis.
