摘要

从“气火相关”的角度探讨慢性阻塞性肺疾病（COPD）的基本病机特点及分阶段防治思路。认为“气火失衡”是COPD的核心病机，并提出COPD早期病机为邪阻肺气、郁火伤络，稳定期病机为元气亏虚、阴火内生，急性加重期病机为邪聚火盛、壮火食气。针对不同阶段的病机特点提出分阶段防治策略，即早期疏调气机、透散郁火，稳定期益气生金、制伏阴火，急性加重期泻火破窠、保元固本。

Abstract

This paper explored the pathogenesis of chronic obstructive pulmonary disease （COPD） through the

-fire theory in traditional Chinese medicine （TCM）， proposing a staged therapeutic framework. It is considered that

-fire imbalance is the core pathogenic mechanism underlying COPD. In the early stage， pathogen obstructing lung

， constraint fire damaging collaterals is the main pathogenesis. In the stable stage， the depletion of original

and internal generation of

yin

fire are predominant， while in the acute exacerbation stage， it is progressed to pathogenic fire accumulation and high fever consuming

.Corresponding to these pathogenic patterns， a staged prevention and treatment strategy has been proposed， that is， in the early stage， it is recommended to regulate

and disperse constraint fire； in the stable stage， the method of tonifying

to generate metal while suppressing latent

yin

fire can be used； and in the acute exacerbation stage， it is suggested to eliminate pathogenic fire through purgation while preserving original

and consolidating the root.
