Exploring the Use of Bitter Medicinals in Chronic Kidney Disease Based on the Theory of Yin Fire
fire is an important component of LI Dongyuan's theory of spleen and stomach. Its main treatment method is to tonify the center
qi
, raise clear
yang
, and drain
yin
fire. The rational use of bitter medicinals is an important part of the clinical prescription and medication based on
yin
fire theory. This article elucidates the complex pathogenesis of chronic kidney disease (CKD) during the evolution of spleen and kidney
qi
deficiency or
yang
deficiency syndrome, including the disorder of
qi
ascending and descending, dampness and turbidity accumulation transforming into heat, and excessive floating of deficient fire, from the perspective of
yin
fire theory. Bitter medicinal herbs can eliminate dampness, drain fire, direct the turbid downward and strengthen
yin
, and can regulate spleen and stomach functions and effectively clear and subdue
yin
fire. Moreover, this article systematically summarized the combination rules of bitter medicinal herbs, such as the combination of the sweet and the bitter, the promotion of
qi
by the pungent and the descending by the bitter, and the combination of the bitter and the
cold with warming
yang
or nou-rishing earth. Furthermore, it is suggested that the selection of bitter medicinals should take into account of the cold, hot, warm, and cool properties of the herbs, as well as their tendency of ascending, descending, floating and sinking, while adhering to the principles such as channel entry and dosage regulation.
