Construction and Validation of Prognostic Model of Elderly Non-small Cell Lung Cancer under Chinese Medicine Intervention
To investigate the prognostic factors of elderly patients with non-small cell lung cancer (NSCLC) under the intervention of traditional Chinese medicine (TCM) in the real world and to construct and validate a survival prognosis model.
Methods
2
The medical records of lung cancer patients from four research centers were retrospectively analyzed. The clinical data of NSCLC patients aged 65 and above who received TCM treatment were collected and divided into the training set, the internal validation set and the external validation set. Based on the training set data, univariate Cox regression, LASSO regression and multivariate Cox regression analyses were conducted to identify independent prognostic factors, and a nomogram model was constructed to predict the 1-year, 3-year and 5-year survival probabilities of elderly NSCLC patients. The model efficacy was evaluated by using C-index (C-Index), calibration curve, area under the receiver operating characteristic curve (AUC), decision analysis curve (DCA), and risk stratification.
Results
2
A total of 1093 patients were included, with 624 in the training set, 268 in the internal validation set, and 201 in the external validation set. A nomogram prognostic model was constructed, which included seven independent prognostic factors, such as TCM syndrome type, TCM decoctions, age, TNM stage, Karl-Heinrich functional status (KPS) score, C-reactive protein and lactate dehydrogenase. The C-indices were 0.776 (95%
CI
: 0.752, 0.801), 0.723 (95%
CI
: 0.683, 0.763), and 0.662 (95%
CI
: 0.616, 0.708) in sequence of the training set, internal validation set and external validation set; the AUC for 1-year, 3-year and 5-year survival rates of th
e training set, internal validation set and external validation set was 0.794, 0.822 and 0.778, 0.735, 0.798 and 0.778, 0.649, 0.693 and 0.638 respectively. The calibration curve indicates that the model has a good degree of calibration. The DCA suggests that the model has a net clinical benefit. The survival differences between the high-risk and low-risk groups were statistically significant (
P
<0.05).
Conclusion
2
This prognostic model developed for elderly NSCLC patients can effectively evaluate patient outcomes and provide a basis for clinical decision-making.
