摘要

总结运用大黄治疗泌尿系统疾病的经验。认为大黄主要有通腑泻下、荡涤肠胃、推陈致新、通利水谷、分消湿热、行瘀决壅、消痈散滞之功，其中生大黄苦寒攻下，酒大黄宣散上行，大黄炭除滞止血，熟大黄缓泻和胃。临证可将大黄不同炮制品灵活配伍或组成经方治疗泌尿系统疾病，如与甘草配伍组成大黄甘草汤加减以通腑泄浊、和胃健脾，治疗慢性肾衰竭；与桃仁、桂枝配伍组成桃核承气汤加减以逐瘀活血、通窍清上，治疗慢性肾脏病合并认知功能障碍；与茵陈五苓散配伍合用以燥湿利水、泻热化瘀，治疗乙肝病毒相关性肾炎；与牡丹皮、桃仁配伍组成大黄牡丹汤加减以行瘀消痈、散结除滞，治疗慢性前列腺炎。

Abstract

This paper summarizes clinical experience in the use of Dahuang （

Radix et Rhizoma Rhei

） for urinary system diseases. Dahuang is primarily believed to exert the following therapeutic effects， promoting bowel movement and purgation， cleansing the gastrointestinal tract， facilitating metabolic renewal， regulating water and grain metabolism， resolving damp-heat， activating blood circulation to remove stasis， and dispersing abscesses and stagnation. Different preparations of Dahuang have distinct properties. Raw Dahuang is bitter and cold and acts as a purgative； wine-processed Dahuang has dispersing and ascending effects； charred Dahuang removes stagnation and stops bleeding； cooked Dahuang gently purges and harmonizes the stomach. In clinical practice， different processed forms of Dahuang can be flexibly combined or used in classical formulas to treat urinary system diseases. For example， Dahuang combined with Gancao （

Radix et Rhizoma Glycyrrhizae

） forms

Dahuang Gancao Decoction

（大黄甘草汤）， and its modifications may promote bowel movement， drain turbidities， and harmonize the stomach and strengthen the spleen for chronic renal failure； Dahuang combined with Taoren （

Semen Persicae

） and Guizhi （

Ramulus Cinnamomi

） forms

Taohe Chengqi Decoction

（桃核承气汤）， and its modifications may activate blood circulation， remove stasis， and clear the upper wh

ile purging the lower for chronic kidney disease with cognitive impairment； Dahuang combined with

Yinchen Wuling Powder

（茵陈五苓散） may dry dampness， promote urination， purge heat， and resolve stasis for hepatitis B virus-associated nephritis； Dahuang combined with Mudanpi （

Cortex Moutan

） and Taoren forms

Dahuang Mudan Decoction

（大黄牡丹汤）， and its modifications may promote blood circulation， disperse abscesses， dissolve nodules， and remove stagnation for chronic prostatitis.
