摘要

目的

分析甲状腺功能减退症（甲减）患者的舌象客观化特征，并探讨舌象特征与甲减的相关性。

方法

采用横断面调查方法，纳入甲减患者200例（甲减组）、健康受试者200例（健康组）。使用Hyper-TCM型舌象成像仪采集两组的舌象图像，观察舌象的舌质（舌色、舌形、老嫩）及舌苔，并分别比较其RGB颜色空间分量（r，红色；g，绿色；b，蓝色）、HSV颜色空间分量（h，色调；s，饱和度；v，明度）、纹理特征参数u和形状特征参数（c，舌体长宽比；l，齿痕点个数），通过二元Logistic回归分析方法分析舌体特征参数与甲减的关系。

结果

甲减组淡白舌46例（23.0%）、暗红舌62例（31.0%）、瘦长舌52例（26.0%）、嫩舌117例（58.5%），比例高于健康组（

＜0.05）。两组受试者舌象特征参数比较，甲减组的r值高于健康组（

＜0.05）、c值低于健康组（

＜0.05）、l值高于健康组（

＜0.05）。两组受试者舌象特征参数回归分析，特征r与甲减呈正相关（回归系数0.023，

=0.007，

=1.024）；特征c与甲减呈负相关（回归系数-2.277，

=0.001，

=0.103）；特征l与甲减呈正相关（回归系数0.095，

＜0.001，

=1.099）。

结论

甲减患者舌象主要特征为舌色淡白/暗红、舌体胖大边有齿痕，部分患者表现舌体瘦长；舌体颜色越淡白/越暗红、舌体齿痕越明显、舌体越胖大，患甲减的几率越高。

Abstract

Objective

To analyze the objective characteristics of tongue images in patients with hypothyroidism and to explore the correlation between tongue features and hypothyroidism.

Methods

A cross-sectional study was conducted including 200 patients with hypothyroidism （hypothyroidism group） and 200 healthy participants （control group）. Tongue images of both groups were captured using the Hyper-TCM tongue imaging system to observe the tongue body （tongue color， tongue shape， tough and tender） and tongue coating. The RGB （r： red， g： green， b： blue） and HSV （h： hue， s： saturation， v： value） color space components， texture feature parameter u， and shape feature parameters （c： tongue length-width ratio； l： number of tooth-mark points） were compared between groups. Binary logistic regression analysis was used to assess the relationships between tongue features and hypothyroidism.

Results

In the hypothyroidism group， pale tongue （46 cases， 23.0%）， dark red tongue （62 cases， 31.0%）， slender tongue （52 cases， 26.0%）， and tender tongue （117 cases， 58.5%） were more prevalent compared with the control group （

＜0.05）. Regarding quantitative tongue image parameters， the r value was higher （

＜0.05）， c value was lower （

＜0.05）， and l value was higher （

＜0.05） in the hypothyroidism group than those in the control group. Logistic regression analysis showed that r was positively correlated with hypothyroidism （

=0.023，

=0.007，

=1.024）； c was negatively correlated （

=-2.277，

=0.001，

=0.103）， and l was positively correlated （

=0.095，

＜0.001，

=1.099）.

Conclusion

The main tongue features of hypothyroid patients are pale or dark-red， enlarged tongue body with tooth marks on the edges， and， in some cases， a slender tongue； the paler or darker the tongue color， the clearer tooth marks， and the more enlarged the tongue body， the higher the likelihood of hypothyroidism.
