Therapeutic Approach to Malignant Pleural Effusion in Lung Cancer Based on the Transmission and Transformation of Damp Turbidity through the Sanjiao (三焦)
This paper proposes that the core pathogenesis of malignant pleural effusion in lung cancer lies in "accumulation of damp turbidity in the lower
sanjiao
(三焦), retrograde transmission through the
sanjiao
, and upward invasion of the lung collaterals". This theory elucidates the pathophysiological process of effusion formation as follows: damp turbidity first accumulates in the lower
sanjiao
, where dysfunction of the kidney and bladder in
qi
transformation serves as the source of disease; it then transmits retrogradely through the
sanjiao
, with stagnation of the
shaoyang
pivot acting as the key link; finally, the damp turbidity ascends to the lung collaterals, where obstruction of chest
yang
leads to fluid retention in the thoracic cavity. Accordingly, the therapeutic principles are established as follows: warming the kidney and promoting
qi
transformation to strengthen the root, by tonifying kidn
ey
yang
and faci-litating bladder
qi
transformation to block the generation of damp turbidity; dredging and unblocking the
sanjiao
to interrupt its pathological transmission, by regulating water pathways and preventing upward encroachment of damp turbidity; and opening and dispersing the upper
sanjiao
to eliminate retained fluids, by relieving obstruction of the lung collaterals. This approach provides a novel perspective for the syndrome differentiation and treatment of malignant pleural effusion in lung cancer.
