摘要

“体脏合痹”理论强调风湿免疫病体痹、脏痹的动态转变特征。以气血津液为立足点，从时间、空间传变规律阐释风湿免疫病“体脏合痹”的内在机理，生理状态下气血津液共同维持体脏间的濡养与气化；病理状态下气机阻滞、津停血瘀、输布失司乃体脏合痹实证病机，气血不足、津伤化燥、生化乏源乃体脏合痹虚证病机，痰瘀互结、浊毒蓄积、代谢失常乃体脏合痹变证病机。依据疾病演变时间进程将风湿免疫病总结为初期气机郁滞、进展期湿痰瘀结、后期浊毒正虚三个病理阶段，同时结合疾病病位传变规律解析体痹及脏、顺传进展，脏痹及体、逆传危急，体脏合痹、顺逆交织的空间传变特征，并提出截断病势、透邪外出，固本培元、调和气血，和解枢机、化浊解毒的治则，有利于指导风湿免疫病的临床诊疗。

The "

（痹） of both body and viscera" theory emphasizes the dynamic transmission characteristics of body

and viscera

in rheumatic and immune diseases. Based on the principles of

， blood， and body fluids， this study elucidates the intrinsic mechanism of the "

of both body and viscera" in rheumatic and immune diseases through temporal and spatial transmission patterns. Under physiological conditions，

， blood， and body fluids jointly maintain the nourishment and

transformation between the body and organs. Under pathological conditions，

movement obstruction， fluid retention and blood stasis， and loss of administration and distribution are the pathological mechanisms of the excess pattern；

and blood insufficiency， fluid damage transforming into dryness， and loss of generation and transformation are the pathological mechanisms of the deficiency pattern； binding of phlegm and stasis， accumulation of turbid toxins， and metabolic disorders are the pathological mechanisms of the transmuted pattern. Based on the progression of the disease， the pathological stages of rheumatic and immune diseaseas are constructed， including

constraint and stagnation in the early stage， dampness phlegm stasis in the advanced stage， and turbidity toxin and healthy

deficiency in the later stage. Combined with the transmission law of the disease of location， the spatial transmission characteristics are formed， which are body

affecting viscera （sequential transmission）， viscera

affecting body （reverse transmission）， and

of both body and viscera （mixed sequential and reverse transmission）. The treatment principles are cutting off the disease trend， penetrating pathogen out， consolidating the root and banking up original

， harmonizing

and blood， resolving the central mechanism， dispelling turbidity and resolving toxins， which are beneficial for guiding the clinical diagnosis and treatment of rheumatic and immune disease.
