摘要

基于“体脏合痹”理论，以态靶辨治方法为指导，提出类风湿关节炎可出现病肢节的“体痹”和病脏腑的“脏痹”表现，病程发展可分为痹态、尪态、虚态三个阶段，以瘀血阻络贯穿疾病始终。痹态包括风湿痹阻证、寒湿痹阻证、湿热痹阻证，尪态包括痰瘀痹阻证，虚态包括气血两虚证、肝肾不足证。治疗需把握“和正调衡”的总体原则，以体养脏、调脏安体、体脏同治，同时分期识态，据态打靶。

Abstract

Based on "

（痹） of both body and viscera" theory and guided by the state-target differentiation and treatment approach， it is proposed that rheumatoid arthritis （RA） may present with manifestations of "body

bi"

（lesions of limbs and joints） and "viscera

bi"

（lesions of

zang

organs）. The disease progression can be divided into three stages， including

state，

wang

（尪） state， and deficiency state， with blood stasis obstructing the collaterals as a core pathological mechanism throughout.The

state includes wind-damp

impediment syndrome， cold-damp impediment syndrome， and damp-heat impediment syndrome； the

wang

state is characterized by phlegm-stasis impediment syndrome； and the deficiency state includes

and blood deficiency syndrome and liver-kidney insufficiency syndrome. Treatment follows the overall principle of harmonizing and regulating the vital balance， emphasizing the strategies of nourishing the viscera through the body， regulating the viscera to stabilize the body， and treating both body and viscera simultaneously. Moreover， staged treatment should be taken according to different state， and applying targeted interventions according to the identified state.
