Modified Shentong Zhuyu Decoction(身痛逐瘀汤) as an Adjuvant Therapy for Rheumatoid Arthritis with Interstitial Lung Disease and Blood Stasis Obstruction Syndrome:A Prospective Cohort Study
To investigate the clinical efficacy and safety of modified
Shentong Zhuyu Decoction(
身痛逐瘀汤, STZYD) as adjuvant therapy in rheumatoid arthritis with interstitial lung disease(RA-ILD) patients and blood stasis obstruction syndrome, and to explore the possible mechanism.
Methods
2
Sixty-five RA-ILD patients with blood stasis obstruction syndrome were enrolled and divided into control group(34 cases) and treatment group(31 cases) based on patients preference. The control group received standard western medicine, while the treatment group received oral STZYD additionally, both for 24 weeks. Before and after treatment, Health Assessment Questionnaire-Disability Index(HAQ-DI), Disease Activity Score-28(DAS 28), and high-resolution computed tomography(HRCT) quantitative score were assessed, as well as the pulmonary function indicators including forced vital capacity(FVC) and diffusing capacity for carbon monoxide(DLCO) as a percentage of predicted normal values. Besides, erythrocyte sedimentation rate(ESR), C-reactive protein(CRP), serum soluble receptor for advanced glycation end products(sRAGE) and sialylated carbohydrate antigen(KL-6) levels were measured. Clinical efficacy and safety were evaluated after treatment. A logistic regression model was used to analyze the impact of oral modified STZYD on clinical efficacy.
Results
2
Four cases in the treatment group, one case in the control group dropped out with 30 patients per group were analyzed. The total clinical effective rate in the treatment group was 96.67%(29/30), significantly higher than 76.67%(23/30) of the control group(
P
<0.05). Both groups showed significant reductions in HAQ-DI, DAS28, CRP, ESR, KL-6 levels, and increases in FVC%, DLCO% and sRAGE after treatment(
P
<0.01), with more improvements in the treatment group than in the control group(
P
<0.05 or
P
<0.01). The lung HRCT score in the treatment group significantly decreased after treatment, which was also lower than the score in the control group(
P
<0.01). In contrast, the control group showed no statistically significant difference in HRCT score after treatment(
P
>0.05). The incidence of adverse reactions was 6.67%(2/30) in the control group and 3.33%(1/30) in the treatment group, with no significant difference between groups(
P
>0.05). Logistic regression analysis showed a possitive correlation between oral modified STZYD and clinical efficacy(
OR
=0.114,95%
CI
: 0.018~0.722,
P
=0.021).
Conclusion
2
The adjunctive use of modified STZYD in RA-ILD patients with blood stasis obstruction syndrome is beneficial in alleviating clinical symptoms, improving pulmonary function, and reducing disease activity. It enhances clinical efficacy with a favo-rable safety. The mechanism may involve reducing inflammation, increasing serum sRAGE levels, and decreasing KL-6 levels.
