(参榆灌肠方, SGF) enema combined with oral mesalazine in the treatment of left-sided ulcerative colitis (UC) with large intestine damp-heat syndrome, and to explore its possible mechanism of action through network pharmacology.
Methods
2
In a prospective cohort design, a total of 60 patients diagnosed with left-sided UC in the mild-moderate active stage were enrolled and were divided into an observation group and a control group according to patients' willingness, with 30 participants in each arm. Both groups received oral mesalazine sustained-release granules at 3 g daily. The observation group was additionally treated with SGF by enema once daily, while the control group received mesalazine suppositories per rectum one suppository daily. The treatment course lasted 8 weeks. The primary efficacy endpoint was the clinical remission rate, and the secondary endpoints included the Mayo score, the scores of individual symptoms in traditional Chinese medicine (TCM), the inflammatory bowel disease questionnaire (IBDQ) score, and the inflammatory bowel disease disability index (IBD-DI) score. Adverse events and adverse reactions were recorded throughout the study. Network pharmaco-logy was used to analyze the active components and key targets of SGF, exploring its potential mechanisms of action, and applying molecular docking to predict the binding activity between the core active components of SGF and the key targets.
Results
2
After 8 weeks of treatment, the clinical remission rate was 86.7% (26/30) in the observation group, higher than 76.7% (23/30) in the control group, while the difference was not significant (
P
>0. 05). The observation group had lower partial modified Mayo scores, bloody stool symptom score, and IBD-DI scores than the control group (
P
<0.05). No seve
re adverse events or reactions were observed during the treatment of both groups. Based on network pharmacology screening, the top five core active components of SGF for UC treatment were quercetin, kaempferol, luteolin, β-sitosterol, and stigmasterol; and the top five key protein targets were tumor protein p53 (TP53), interleukin-6 (IL-6), serine/ threonine kinase 1 (AKT1), interleukin-1β (IL-1β), and epidermal growth factor receptor (EGFR).
Conclusion
2
SGF enema combined with oral mesalazine can effectively alleviate clinical symptoms, improve quality of life and reduce the disability index in left-sided UC patients with large intestine damp-heat syndrome, with favorable safety. SGF may exert its function through cancer pathways and inflammatory pathways, and quercetin, kaempferol, and other compounds may be the core active ingredients for treating UC.
