摘要

目的

评价保和丸方加减联合常规肺康复治疗对肺癌电视辅助胸腔镜手术（VATS）后患者胸腔引流液量、胸腔引流管留置时间及外周血炎症指标的影响。

方法

采用随机平行对照研究设计，纳入肺癌VATS后患者118例，治疗组和对照组各59例。对照组接受常规肺康复治疗，治疗组在此基础上口服保和丸方加减汤剂，治疗至拔管出院。主要结局为术后胸腔总引流液量，次要结局为胸腔引流管留置时间及外周血炎症指标［包括血小板与淋巴细胞比值（PLR）、中性粒细胞与淋巴细胞比值（NLR）、淋巴细胞与单核细胞比值（LMR）、全身免疫炎症指数（SII）、全身炎症综合指数（AISI）及全身炎症反应指数（SIRI）的治疗前后水平和相对变化值］。记录两组治疗期间的不良事件。

结果

治疗组术后总引流液量为［850（430，1410）］ml，低于对照组的1085［（660，1720）］ml（

＜0.05）。治疗组术后胸腔引流管总留置时间为8［（6，11）］天，短于对照组的［10（7，11）］天（

＜0.05），但入组后拔管时间组间差异无统计学意义（

＞0.05）。校正术后至入组时间、入组前引流液量及观察时长后，两组术后总留置时间差异无统计学意义（

＞0.05），而入组后累计引流液量治疗组仍低于对照组（

＜0.05）。治疗后治疗组NLR小于对照组，NLR相对变化值大于对照组（

＜0.05），其他炎症指标的治疗后水平及相对变化值组间差异均无统计学意义（

＞0.05）。研究期间两组未报告与治疗相关的严重不良事件。

结论

在常规肺康复治疗基础上加用保和丸方加减，可减少肺癌VATS术后患者入组后胸腔引流液量，且安全性较好，对术后胸腔引流管总留置时间及外周血炎症指标的影响尚需大样本、多中心研究验证。

Abstract

Objective

To evaluate the effects of modified

Baohe Pill

（保和丸） formula added to conventional pulmonary rehabilitation on thoracic drainage volume， chest tube duration， and peripheral blood inflammatory indices after video-assisted thoracoscopic surgery （VATS） for lung cancer.

Methods

In this randomized parallel-controlled study， 118 post-VATS patients were enrolled， with 59 cases in each group. The control group received conventional pulmonary rehabilitation， while the treatment group additionally received an oral decoction of modified

Baohe Pill

formula until chest tube removal and discharge. The primary outcome was total postoperative thoracic drainage volume， while the secondary outcomes included chest tube duration and inflammatory indices including the pre-treatment and post-treatment levels and relative changes in platelet-to-lymphocyte ratio （PLR）， neutrophil-to-lymphocyte ratio （NLR）， lymphocyte-to-monocyte ratio （LMR）， systemic immune-inflammation index （SII）， aggregate index of systemic inflammation （AISI）， and systemic inflammation response index （SIRI）. A

dverse events during the treatment period were recorded.

Results

The total postoperative thoracic drainage volume was 850 （430， 1410） ml in the treatment group， lower than 1085 （660， 1720） ml in the control group （

＜0.05）. Total postoperative chest tube duration was 8 （6， 11） days in the treatment group， shorter than 10 （7， 11） days in the control group （

＜0.05）， whereas the interval from enrollment to removal did not differ significantly between groups （

＞0.05）. The postoperative NLR in the treatment group was lower than that in the control group， and the relative change in NLR was greater than that in the control group （

＜0.05）. No significant between-group differences were found in post-treatment levels or relative changes of the other five inflammatory indices （

＞0.05）. No treatment-related serious adverse events were reported in either group.

Conclusion

Modified

Baohe Pill

formula plus conventional pulmonary rehabilitation has lower total postoperative thoracic drainage volume， shorter total postoperative chest tube duration， and lower NLR levels in lung cancer patients following VATS， while demonstrating a more favorable safety profile than conventional pulmonary rehabilitation alone.
