摘要:By systematically analyzing the bottlenecks in current traditional Chinese medicine (TCM) efficacy evaluation, this study addresses the limitations of TCM efficacy assessment methods in terms of individualization, holism, and complexity representation. Drawing on the concepts and methodologies of evidence-based medicine and systems biology, this paper proposes a new evaluation path by integrating multidisciplinary technologies such as big data, artificial intelligence (AI), omics techniques, and bioinformatics, and encompassing from foundational data, individualized evaluation, and mechanistic analysis to evidence translation. The path first integrates real-world data, multi-omics profiling, and dynamic monitoring via smart devices to construct a high-dimensional dynamic data foundation. Subsequently, leveraging this foundation, it combines digital twins, dynamic symptom networks, and reinforcement learning to develop a dynamic efficacy evaluation method based on individual self-reference, enabling a shift from group mean comparisons to individual potential outcome comparisons. Furthermore, adopting the systems biology paradigm, it employs dynamic omics monitoring and deep learning models to dissect the synergistic mechanisms of TCM compound prescriptions from network regulation, emergent effects, and dynamic evolution dimensions. Finally, it enhances real-world evidence strength through target trial emulation and causal inference, achieving model iteration via incremental learning. This path aims to advance efficacy evaluation from "point validation" to "systematic characterization" and from "outcome assessment" to "process mechanism elucidation," providing methodological support for the modernization of TCM.

摘要:Traditional Chinese medicine (TCM) diagnosis is a cognitive process where practitioners rely on their subjective clinical experience and clinical perception abilities to comprehensively integrate the four examinations and flexibly differentiate syndromes, embodying a simple humanistic concern for the state of life. The essential characteristics of the subjectivity and experience of TCM diagnosis determine that there is a threshold in the syndrome differentiation capability of the artificial intelligence (AI)-based TCM diagnostic systems, which refers to a critical point of capability in the process of simulating the syndrome differentiation thinking, due to its inability to replicate the subjective cognition, clinical perception, and humanistic care of the practitioners. Furthermore, this threshold encompasses both a technical threshold and a theoretical threshold. Based on the above analysis, four human-machine collaborative practice paths are proposed to balance the application of AI technology and the subjectivity of TCM syndrome differentiation, including clarifying the auxiliary diagnosis and treatment positioning of AI, optimizing algorithms to achieve technical adaptation, strengthening the cultivation of the practitioner's subjectivity, and promoting interdisciplinary cross-collaboration, aiming to provide theoretical references for the development of AI-based TCM diagnostic systems towards human-machine collaborative clinical applications.

摘要:This paper summarizes Professor WU Mianhua's clinical experience in the staged treatment of radiation-induced brain injury (RIBI) based on the stasis-heat theory. It is believed that radiation fire-toxin is the initiating pathogenic factor of RIBI, and the core pathogenesis consists of the intermingling of stasis and heat, obstruction of collaterals by wind-phlegm, dual deficiency of qi and yin, as well as impairment of essence and depletion of marrow. In the early stage, the disease is mainly manifested as stasis-heat with fluid retention and obstruction of collaterals by wind-phlegm, with the lesion located in the brain collaterals. The therapeutic principle is to cool the blood and dissipate stasis, extinguish wind and unblock collaterals, and simultaneously eliminate phlegm and promote urination, according to which self-designed Liangxue Tongluo Formula (凉血通络方) is selected for treatment. In the middle stage, the key pathological features are the binding of phlegm, stasis and heat, and consumption of kidney essence, with the lesion invading the kidney essence. The therapeutic principle is to resolve phlegm and remove stasis, clear and relieve latent heat, replenish essence and tonify marrow, and the self-designed Qinghua Tianjing Formula (清化填精方) is selected for treatment. In the advanced stage, the disease is mostly manifested as dual deficiency of qi and yin and toxin-induced marrow exhaustion, with the lesion involving the brain marrow. The therapeutic principle is to replenish qi and nourish yin, remove toxin and tonify marrow, and the self-designed Yangyin Jiedu Formula (养阴解毒方) is selected for treatment.

摘要:This article summarizes Professor HAN Mingxiang's clinical experience in treating lung cancer based on the theory of "deficiency-phlegm-stasis-toxin". It is believed that depletion of healthy qi, together with binding of phlegm and stasis forming into toxin is the core pathogenesis of lung cancer occurrence and development. In clinical practice, the core treatment principle should be reinforcing healthy qi, resolving phlegm, removing stasis, and relieving toxins. The treatment should be given based on syndrome differentiation. For the syndrome of lung-spleen qi deficiency, the self-formulated Jianpi Huatan Zhike Decoction (健脾化痰止咳汤) can be applied to fortify the spleen and replenish qi, thereby reinforcing healthy qi. For the syndrome of deficiency of both qi and yin, the self-formulated Shenmai Yuzhu Decoction (参麦玉竹汤) is used to nourish yin and moisten the lungs, thereby clearing the source. For the syndrome of phlegm-dampness accumulating in the lungs, the self-formulated Sizi Wenfei Decoction (四子温肺汤) is applied to warm the lungs and resolve phlegm, thereby diffusing the lung qi. For the syndrome of qi stagnation and blood stasis, the self-formulated Yiqi Zhuyu Decoction (益气逐瘀汤) is used to activate blood and resolve stasis, thereby unblocking the collaterals. For the syndrome of binding of phlegm and stasis, the self-formulated Jiejie Decoction (结节方) is utilized to resolve phlegm and remove stasis, thereby dissipating nodules. For the syndrome of lung deficiency with toxin accumulation, the self-formulated Qiyu Sanlong Decoction (芪玉三龙汤) is applied to reinforce healthy qi and remove toxins, thereby combating cancer. Medications are flexibly prescribed based on syndrome differentiation, with an emphasis on reinforcing healthy qi and eliminating pathogenic factors, and maintaining the balance of zang-fu (脏腑) organ functions as well as qi, blood, yin and yang.

摘要:Specification for Quality Control in Traditional Chinese Medicine Clinical Trials(GB/T 47336-2026) is the first national standard in the field of clinical research on traditional Chinese medicine (TCM), aims to address the core issue that the quality shortcomings of current TCM clinical research restrict the transformation of achievements and international recognition. Through the interpretation of the core contents of this standard, including its scope of application, graded quality control system, TCM-specific quality control requirements, implementation and responsible entities, responsibilities of quality control personnel, formulation of quality control plans, critical nodes of quality control, and quality control archive management, it is believed that the promotion and implementation of this standard are of great significance for improving the standard system of TCM clinical research, enhancing research quality, optimizing the quality management model, and promoting the internationalization process of TCM. Meanwhile, the implementation paths for the standard are discussed, including carrying out pilot applications in institutions, establishing publicity, implementation, and training mechanisms, encouraging multi-party collaborative supervision, and setting up implementation feedback mechanisms. On this basis, the challenges that may be encountered during implementation, such as resource input pressure, difficulties in implementing TCM-specific quality control, and a relative shortage of interdisciplinary composite talents, are sorted out, and corresponding strategies are proposed, which can provide references for relevant researchers and practitioners.
