Objective To observe the therapeutic effect and mechanism of Shunaoxin Dropping Pill for acute ischemic stroke（AIS）with blood stasis pattern.Methods Totally 154AIS patients with blood stasis pattern were randomized into the control group（81cases

conventional therapy）and trail group（73cases

conventional therapy+Shunaoxin Dropping Pill）.The treatment lasted for 14days.The scores of TCM symptom scale and National Institutes of Health Stroke Scale（NIHSS）were evaluated on the admission day and after treatment.The platelet count（PLT）

activated partial thromboplastin time（APTT）

thrombin time（TT）

fibrinogen（FIB）

soluble P-selectin（sP-sel）and soluble CD40L（sCD40L）were detected.Results The TCM symptom scale score

NIHSS score and MPV were significantly decreased in both groups after treatment（P<0.05or P<0.01）.The platelet aggregation rate

sP-sel and sCD40Lwere significantly decreased but the APTT were significantly increased in the trail group after treatment（P<0.05or P<0.01）.After treatment

the NIHSS score and platelet aggregation rate in the trail group were significantly lower than those in the control group

but the APTT in the trail group was significantly higher than that in the control group（P<0.05or P<0.01）.Conclusion Shunaoxin Dropping Pill can relieve the TCM symptoms and neurological deficit that may be relevant to inhibiting the platelet activation and aggregation and intervening the intrinsic coagulation pathway.
